Miscellaneous

  • Required portable fire-fighting equipment available properly located and maintained?

Personal Protection (PPE)

Ladders

Scaffolding

Rolling Scaffolds (Manually Propelled)

  • Work levels 6' or more above ground or floor level have guard rails and toe boards?

  • Height does not exceed 4 times least base dimension unless outriggers are used?

Floor and wall openings

  • Wall openings (30" high, 18" wide or greater) from which there is a drop of 4' or more and bottom of opening less than 3' above working surface provided with guard rails

  • Open sided floors 6' or more above floor or ground level provided with standard railing and toe board or other equivalent perimeter protection?

Roofing

Flat and Low Pitch Roofs Less than 4/1 Pitch

  • Warning line 6' from roof edge in the work area?

  • Safety belts and lines in use at perimeter or standard handrails or equivalent?

  • Protective equipment used when handling hot pitch?

  • All openings covered or protected by guardrails?

Concrete forming and pouring

  • Vertical re-steel protected by covering when employees working above?

  • Employees provided with and using PPE while pouring concrete?

  • Shoring and re-shoring adequate for support?

Steel Erection

  • Decking or safety nets 2 stories or no more than 30' where erection is being performed

  • NO riding of steel or the hook

  • Safety railing of no less than 1/2" wire rope 42" high around perimeter of all temporary planked or metal decked floors.

  • Employees working in float scaffolds provided with and using safety belts?
