Field Safety Audit

Job Essentials

  • All LPC employees attending shift change customer safety meeting.

  • All LPC employees know where primary and secondary muster points are located.

  • JSA completed for operations.

  • Command center clean and in good order.

  • LPC employees are observing customer requirements regarding jewelry, hair wear, facial hair, etc.

  • No tripping hazards exist due to LPC equipment.

  • Potable water is available for hydration.

  • Employees are taking necessary breaks and staying hydrated.

  • Heating/cooling systems in work areas are functioning properly.

  • All necessary handrails installed and entryways are clear for safe entry and egress from work stations.

  • All work areas have adequate ventilation.

  • All work areas have adequate lighting.

  • SDS available for all chemicals LPC has on location.(Should be in binder).

  • Spill kit(s) available.

Personal Protection Equipment (PPE)

  • All personnel wearing required PPE.

  • All PPE being worn correctly(FRC sleeves not rolled up, clear safety glasses at night, etc.)

  • Proper hearing protection being worn when employee(s) exposed to 85DB or greater noise level.

  • Hard hats are all within date per manufacturer's guidelines.

  • Wearing impact gloves if required by customer.

  • All LPC employees have H2S monitors that are active and functioning properly (if required).

  • All LPC employees are wearing H2S monitors properly in the breathing zone outside of clothing. (if required).

  • PPE inventory stocked to include surplus of hearing protection, gloves, safety glasses, etc.

Electrical Safety

  • LOTO binder available, utilized & current.

  • LOTO apparatuses upgraded, installed and functioning for pump LOTO.

  • Necessary labels concerning electrical hazards are posted on equipment and easily visible.

  • Temporary wiring and/or extension cords are placed so that they do not create a tripping hazard.

  • Power shut offs are within easy reach of operators and in good working order.

  • Command center & generator grounded properly.

High Pressure Area (RED ZONE)

  • Flagging or safety chain up designating red zone are of operations.

  • High pressure red zone signs are displayed designating "no entry area".

Vehicles/Pumps/Trailers

  • Iron restraints are installed properly to include incorporating chiksan closest to pumps.

  • Pins are installed camlock connections (and taped at connection if customer requires).

  • All iron connections are made up securely and not leaking.

  • All LPC crew vehicles parked per customer's requirement.

  • All vehicles are in proper working condition with all required lights, signals, mirrors, etc. installed and functioning properly.

  • All vehicles and equipment have wheel chocks in place.

First Aid

  • First Aid kit(s) available, in date and stocked.

  • Eye wash station available and in good working order(if customer required).

  • Emergency Response Plan posted to include nearest medical facility & emergency contact numbers.

Fire Extinguishers

  • Fire extinguishers properly placed in front of tractor pumps.

  • Fire extinguishers in place for test pump.

  • Fire extinguisher in place on grease unit.

  • Proper size fire extinguishers for equipment.

  • Fire extinguishers in cert with inspection tag and date clearly visible.

Hand Tools & Power Tools

  • All hand tools are in good working order

  • No "home made" hand tools on location.

  • All power tools are in good working order free from damage or frayed cords.

  • All power tools have required guards and safety features in place and in good working order.

  • Power tool(s) that are damaged, missing parts or broken are tagged and placed out of service for return to shop facility for proper repair or disposal.
