Top Deck Inspection

  • Has ALL WORK been completed and ready for the pour (formwork, rebar, post-tension, embeds, plumbers, electricians)? If not, please describe.

  • Is the formwork deck finished 100% and ready for pour (bulkheads done, edgeforms done, openings installed, etc.) ?

  • Is all rebar installed per the approved shop drawings?

  • Is all rebar supported properly on chairs?

  • Is rebar installed properly to provide proper concrete coverage on all sides?

  • Is post-tensioning laid out and installed correctly per approved shop drawings?

  • Are all embeds laid out and installed per approved shop drawings? If not, please describe.

  • Are all edgeforms kicked off properly and plumb?

  • Is the drip strip installed at all required locations?

  • Are all safety rails and toe boards installed properly at slab edge and any interior openings?

  • Is deck free and clear of all debris and ready for concrete pour?

RMD Beam Tables

  • Do you have a stamped shoring drawing onsite for this pour?

  • Is the shoring installed per the stamped drawing? If not, contact PC, OM or Area Lead.

  • Are RMD beam table legs visually plumb?

  • Are all beam tables set at the correct elevation or grade?

  • Is all required Alshor bracing installed per the drawing?

  • On ramps, are hardwood wedges installed under post shore bases?

Long Pan Forms

  • Are all slab tables installed per the drawings with full bearing on the beam tables?

  • In any "loose framed" slab areas, is the shoring installed per the drawing? If no, indicate why not.

  • Are all the slab table inserts installed for stripping purposes?

  • Are all pan forks installed no more than 3ft on center?

  • Are all pans clipped together at the concrete joist bottoms per the drawing?

  • Are all post shores under the long pans installed per the drawing, no more than 3’-6” on center?

Reshoring

  • Does this pour require reshores?

  • Is there a stamped reshore drawing onsite?

  • How many floors of reshore are required?

  • Are the correct type of reshores used and installed snug and plumb?

  • Is reshoring required to be leg-to-leg?

  • Are reshore spring clips in place at top of shore?

  • On steel shores, are the reshore pins completely inserted and the shore snug?

  • Are reshores installed visually plumb?

  • Are perimeter reshores tied with a rope to prevent falling from the building?

  • Do all the floors engaged in shoring and reshoring meet the specified strength requirement per the reshore drawing?

  • Are all hole covers installed 100% to prevent fall hazard and NO reshores installed on hole covers?

  • Is the outrigger reshored at least two levels?

Subgrade Information

  • Is the slab on grade in place?

  • If slab on grade is not in place, is compaction and bearing capacity enough for the leg load?

  • Are the mud sills the proper material and size per the stamped drawing?

  • Have efforts been made to prevent runoff from washing under mud sills during heavy rains?

  • Are all mud sills level, completely bearing on dirt/gravel, and centered underneath the frame base plate?
