Pre Inspection Checks

  • Have vehicle pre-starts been completed for all trailers, vehicles and machines on site?

  • Have any Green Forms been completed for any vehicle faults?

  • For any issues deemed significant, and where the vehicle has been taken, discuss this with Greg or Nick.

  • Are there copies of valid driver licences in the training Smartsheet for all crew member driving our vehicles to, from and at site?

Arrival at site

  • Is appropriate TM in place and was radio contact easily established as you reached the site?

  • Were the instructions from the TM staff clear about where to park?

  • Were you inducted into the site by the STMS?

  • Were TTM staff wearing correct PPE?

  • Is the STMS clearly identifiable

  • STMS must wear a yellow hi viz with STMS in red on it.

Paperwork

  • Has toolbox meeting been held, including all staff and any sub-contractors?

  • Has the DRCP been appropriately completed. Are the the risks and hazards documented relevant? Are there any obvious ones missing?

  • All works must cease as soon as practicable until a Daily Risk Control Plan has been discussed and signed.

  • Does the DRCP list the SWMS that apply to the job - specifically including: Excavator Winch, Working in a Waterway or Body of Water, Breaking Out?

  • Can a randomly nominated crew member confirm the location of the nearest spill kit, fire extinguisher and first aid kit?

  • The site foreman must advise all crew members via radio of the locations (which must also be documented on the DRCP)

  • Can a randomly nominated crew member confirm the location of the site paperwork?

  • Does the site drawing generally reflect the actual site?

  • If close approach permit(s) is required, is a copy of the permit on file?

  • All works that require a permit must cease until a permit is made available on site.

  • Is the Notification of Hazardous Works receipt from WorkSafe on file?

  • All hazardous works must cease until the receipt is made available on site.

  • Have class 4 & 5 truck drivers completed log books?

  • Log books must be completed. Despite some beliefs, Uttings Road is a public road.

Site Conditions

  • Has the site been planned adequately to accommodate plant and equipment, including space for visitor vehicles?

  • Are there designated safe zones for workers? These should be drawn on the site drawing.

  • Has bunting been installed where there are overhead assets in vehicle movement areas?

  • Is machine operation safe for machine and operator, workers and general public i.e. doors closed, good comms between operator and other crew members.

  • Machine operation must be immediately made safe

  • Is a dedicated machine re-fuelling area established and is a spill kit and fire extinguisher readily available in this area? Re-fuelling area must not be near a waterway,

  • A spill kit and fore extinguisher must be readily available at the re-fuelling area when a machine or plant item is re-fuelled.

  • Is the portable toilet located in an appropriate area? Is it suitably provisioned with toilet paper and hand washing water?

Plant and Equipment Maintenance

  • Are beacon lights working on a random selection of vehicles?

  • Are machine horns or other audible alarms working?

  • Are first aid kits and fire extinguishers (current) in a random selection of vehicles?

People

  • Crew member PPE is correct for the task, and is correctly worn?

  • Are duties rotated or breaks scheduled to ensure staff are not unreasonably fatigued?

  • Do crew members have sufficient food and water for the day?

  • Do crew members have access to an area out of the sun or rain to take breaks?

Tree Felling

  • Is tree felling being undertaken by a trained and competent arborist/faller?

  • All felling works but stop and be resumed by a trained and competent arborist/feller

  • Are records of learning held on site for the arborist/feller?

Breaking-out

  • Does the person supervising the works have Unit Standard 1258? Is a copy retained on site?

  • All breaking out works must cease until evidence of the achievement is made available on site.

  • Has the person completing breaking-out duties been deemed (and documented) to be competent by the person with Unit Standard 1258?

  • A record of learning/evidence of competency must be retained on site before the person can undertake the breaker-out duties.

  • Can the spotter maintain visual contact with the machine operator and the breaker-out?

  • The team should reset, ensuring visual contact is maintained.

  • Can the machine operator, spotter and breaker out confirm the emergency procedures?

  • Works must cease until the operator, spotter and breaker-out have familiarised themselves with the emergency procedures section of the SWMS' applicable to the job.

  • Can the breaker out identify the no go zones?

  • Works must cease until the breaker-out is made aware of, and understands, the no-go zones.
