Safety Essential Verification - Working at Heights

  • Take a photo of the working at heights activities

  • After a discussion with the workgroup and supervisor, was performing the work at ground level or pre-fabricating or preparing materials prior to arrival on site considered prior to undertaking the task?

  • Are Above the Line methods being used to treat work at height risk?

  • Take a photo of the area. Identify if the area has the size of exclusion zones physically barricaded around work at height activities been identified to sufficiently protect plant or personnel on the ground from dropped objects?

  • Are horizontal or vertical openings fitted with fixed penetration covers signed with ‘Penetration Below’ and where required, the Safe Working Load of the cover?

  • Are workers who are using fall arrest and restraint equipment attached at all times while at height?

  • Is a reliable means of communication in place

  • Have platform ladders only been used where it is not practical to use a scaffold or MEWP?

  • Are workers that are wearing fall protection equipment familiar with the equipment's inspection process, its correct use and the rescue plan in case of a fall or becoming stuck at height?

  • Are workers aware of the risk of being pinned during MEWP movement and what controls are in place to avoid this risk eventuating?
