Monthly Department Inspection

Housekeeping

  • Floors clean and dry, corners free of clutter, and tops of cabinets not used for storage

  • Is trash being emptied and not overflowing. Cleaning supplies, tools and equipment (trash cans, brooms, and floor sweep to be stored in designated locations.

  • Condition of floors - Floors are clean of all debris, chips, trash dust etc. Floor cleaning should be done on a daily basis.

  • Work carts and Supply Cabinets cleaned and organized. There are no unneeded machines, tools or equipment in area.

  • No open food or drinks in the area. No personal items left in area.

  • Employees not wearing two headphones at the same time?

Emergency Action Plan

  • Is spill kit available and in the correct place?

  • Emergency Access -Safety Equipment- Safety devices are clearly marked, highly visible and unobstructed. Emergency egress routes are marked and exit signs, lights, etc. are in good condition.

  • Is eye wash station covered, clean and overflow bucket empty?

Walking Working Surfaces

  • Are spotters being used when backing up boats or moving molds/dollies the operator can't see around?

  • Supplies- All Supply Cabinets are fully stocked and maintained in a clean, orderly fashion.

  • Are standard guardrails provided wherever aisle or walkway surfaces are elevated 4 feet or more above any adjacent floor or the ground?

  • Are ladders and/or catwalks positioned properly and in good condition. Are employees facing the ladder when ascending and descending ladder.

  • Is excessive debris and unnecessary tripping hazards found in the production areas?

Fire Prevention

  • All fire extinguishers marked and accessible?

  • Flammable materials are stored in flammable cabinet?

  • Are all flammable liquid barrels grounded?

  • Trash dumpster do not contain any "HOT" materials?

  • Combustible materials are being stored away from where MEKP is present. Hazmat Storage-Liquids, Solvents, Flammables, and other chemicals are properly labeled and stored.

  • Amount of MEKP on the floor meets company standard and is stored correctly?

  • All fire prevention containers, barrels, and cabinets are closed? (i.e. hot barrels, acetone containers, etc.)

PPE

  • All employees are wearing clear safety glasses in the production area?

  • Cut resistant glove is worn on opposite hand when using a razor knife/blade?

  • Are chemical resistant gloves being worn when handling chemicals?

  • Is hearing protection being used in required areas?

  • Used razor blades are placed in razor blade container for disposal?

  • Safety- PPE notices are posted in every area and all employees are wearing the required PPE.

  • Is respirators stored in nonporous, sturdy, airtight containers, like a zip-sealed plastic bag or plastic container. Are those wearing a tight fitting respirator clean shaven?

Electrical, Mechanical & Air Systems

  • Are guards installed and being used?

  • Is Jigs, Cradles, Stands, and other equipment in good working order. There are no air or fluid leaks. View windows, guards and deflectors are clean & in place.

  • Extension cords have ground prong and no frays in cord?

  • Are compressed air nozzles at the correct pressure of 30 psi or less, and with OSHA approved air wands?

  • Is there at least three (3) feet clearance in front of electrical panels/breaker boxes?

  • Are all unused openings, including conduit knockouts in electrical enclosures and fittings closed with appropriate covers, plugs, or plates?

  • Are electrical panels closed with no visible damage or exposed wires?

Chemical Process Operations

  • All chemicals stored & labeled properly? Containers are in good condition?

  • Is Acetone stored in closed flammable containers?

  • No signs of leaking MEKP, resin, or other chemicals present?

Waste Management

  • Are used solvent rags being stored in closed containers?

  • Are aerosol cans disposed of in accumulation drum and not in trash cans?

  • Is cardboard, paper, or combustibles in "Hot Containers"?

  • Are all hazardous waste containers labeled correctly and closed?

  • Is trash removed promptly? (cans/dumpsters not overflowing)?

Hoist / Lifting

  • Are slings in good condition? (not frayed, cut or deteriorated)

  • Is safety latches on crane hooks in place and operational?

  • Does spreader bars or other lifting devices have a ID tag present and completed daily?

  • Are the controls of hoist clearly marked to indicate the direction of travel or motion? Is load rating clearly marked and visible?

  • There is no one standing under a load when hoisting material or equipment?

  • Is team lift being used and lifting >50lbs and, is hoist or lifting device being used to lift items >100lbs or greater?

Material Handling and Storaga

  • Are trucks and trailer secured with wheel chocks during loading and unloading operations?

  • Are chains in place across loading dock when not loading or unloading?

Powered Industrial Trucks

  • Has a preshift inspection been conducted on powered industrial trucks?

  • Is operator going a safe speed, sounding horn and wearing seatbelt?

  • Is parking brake applied and forks on ground when parked?

Hot Work/Welding

  • Is compressed gas bottles secured and equipped with "Clamshell" type valve protectors?

  • Is flashback and backflow preventers at both the torch and regulator on oxygen/fuel cutting systems present?

  • Are welding curtains and welding hood being used while welding?
