ELECTRICAL

TOOLS

  • Proper tool being used for job?

  • Are tools being used properly?

  • Are there tools that need to be repaired or replaced?

LADDERS

  • Ladders properly secured to prevent slipping, sliding or falling?

  • Do straight ladders extend 36" above top of landing?

  • Offset protection at top ladder entrance?

  • Stepladders fully open when in use?

  • Is the top or top two steps of a stepladder being used to perform a task?

  • Correct height ladder used for the task being performed?

  • Safe use / Can have three point contact?

  • Proper maintenance (Labels are legible) and storage of ladders?

SCAFFOLDING

  • Is scaffold equipment in good working order?

  • Inspected by competent person daily before use?

  • Erected on solid, level footing?

  • Guardrails on open sided and end more than 6 ft above the ground?

  • 2x4 top rail, mid-rail & toe board?

  • Tightly planked platform for full width and secured in place?

  • Is the scaffold plump and square, with cross-bracing?

  • Access ladder provided?

  • Maximum height = four (4) times minimum base diminsion?

FALL PROTECTION

  • Is 100% Fall Protection required for all heights above 6 ft with proper anchor points?

  • Floor opening two (2) inches and larger covered, anchored and labeled.

  • Roof opening/curbs covered and secure.

  • Roof edge adequately guarded?

  • Safety harness, lifelines and lanyard worn when required/needed.

  • Are employees wearing PFA's harnesses correctly, and are the harnesses the correct size for employees?

AERIAL MANLIFTS/SCISSORS LIFTS/BOOM LIFTS

  • 100 % Fall Protection required for this job site?

  • Restraining protection used in Boom Lifts.

  • All personnel tied off when required?

  • Safety chains in place on scissors lifts?

  • Working Safe in Lifts.

HOISTS AND CRANES

  • Slings and chains , hooks and eyes in good condition and tagged/marked?

  • Equipment firmly supported?

  • Outriggers used if needed and proper cribbing in place?

  • Power lines inactive, removed or at a safe distance? Minimum of 10 feet.

  • Signalmen where needed & hand/verbal signals understood?

  • Crane's Swing radius is protected.

HEAVY EQUIPMENT- Backhoe, Trackhoe, Min-X, Skid Loader

  • Lights, brakes, horn, back up alarm operative?

  • Carbon Monoxide signs post

  • Carbon Monoxide level checked

  • Fire extinguisher installed?

  • Roll Bar are properly installed

  • Seat belts being used?

FORKLIFTS / LULLS

  • Are all operators trained and certified to operate the equipment?

  • Are seat Belts being worn?

  • Carbon Monoxide sign posted

  • Lights, brakes, horn, back up alarm operative?

EXCAVATION AND SHORING

  • Comp. person(s) designated?

  • Underground utilities located (811) or by private services?

  • Are adjacent structures including roads/sidewalks properly shored/supported?

  • Soil Classification.

  • Excavation Depth.

  • Excavation? Protection from cave-ins for >five (5) feet deep?

  • Excavation Width.

  • Excavation Length.

  • Spoils back a minimum of two (2) Feet?

  • Material i.e... Pipe, not stored at trench edge?

  • Means of egress within 25 feet of workers in excavation > four (4) feet deep? Ext. Ladder three (3) feet pass ground level or Ramp used.

  • Is excavation properly sloped, shored or trench box used?

  • Tabulated data for trench box on site?

  • Fall protection/barricading adequate?

  • Is equipment a safe distance from edge of excavation?

HANDLING & STORAGE OF MATERIALS

  • Materials properly stored or stacked?

  • Are loads lifted correctly? Proper number of men or equipment used?

WELDING AND CUTTING/OTHER HOT WORK

  • Screens and shields used where necessary?

  • Goggles, gloves, proper clothing worn?

  • Welding leads and gases hoses protected and in good condition?

  • Fire extinguishers and proper type in reach of ANY and all Hot Work?

  • Are fire Extinguishers located a minimum of one (1) per every 3,000sf throughout project job site?

  • Inspection of fire hazards?

  • Gas cylinders secured and upright. Fuel cylinders at least 20 ft from oxygen?

  • Are cylinder caps in use?

  • Fire watch used where needed/required?

Cylinders

  • Oxygen and fuel cylinders properly separated by one (1) hour fire barrier, or 25 feet space?

  • Are all compressed gas cylinders labeled?

  • Cylinders not in use are stored with valves closed, regulators removed and caps in place?

  • All hoses and torches free from defects?

  • Do "B" tanks have keys on the stem?
