Physical Compliance Observations

General Work Environment / Housekeeping

  • Are all areas of the project site in good condition? Is trash cleaned up? Materials properly staged?

Personal Protective Equipment

  • Is the site required personal protective equipment being worn by all workers on-site (eye protection, high viz. clothing, hardhats, footwear)?

  • Are there work activities that require additional or specialized PPE ( gloves, dust masks, hearing protection, metatarsal guards, face shields, respiratory protection, welding helmets, etc), and are they being worn properly?

Fall Prevention - Guardrails, Handrails, Warning Lines, Holes/Floor Openings, etc.

  • Are there areas requiring guardrails, handrails or warning lines?

  • Are any guardrails, handrails (on 4 or more stairs) or warning lines missing?

  • Are they constructed according to standards and maintained?

  • Are all floor openings guarded by a cover, guard rail?

  • Other Fall Protection issues?

Fall Protection - Personal Fall Arrest Systems

  • Are there current work activities requiring Personal Fall Arrest or Restraint Systems?

  • Is the fall arrest/fall restraint equipment being installed correctly and used properly?

  • Does the PFAS appeared to be in good condition?

  • Is the "competent person" on site and in the work area during PFAS use activities?

  • Other PFAS issues?

Fire Protection, Prevention and Safety

  • Are adequate numbers of fire extinguishers present throughout the worksite (adequate numbers), fully charged, mounted, unobstructed and inspected regularly?

  • Are flammable and combustible materials properly stored, with appropriate "NO SMOKING" signage?

  • Are fire extinguishers present within 20 feet (10 feet for inside storage) of flammable materials and gas powered equipment?

  • Other Fire Safety issues?

First Aid

  • Are there adequate number of first aid kits available and fully stocked according to standards?

  • Are eyewash bottles throughout the project site and the portable eyewash station available and in sanitary condition?

Site Access and Emergency Egress

  • Are exits appropriately signed or marked?

  • Are there adequate means of egress (number and types) and free/clear of obstructions?

  • Are emergency alarms (air horns) or other means of announcing an emergency present?

  • Are project radios being used and in operable condition?

  • Other Emergency Response issues?

Hazardous and Flammable Substances/Materials

  • Are hazardous substances are properly stored and contained?

  • Is the proper personal protective equipment available and being used when handling hazardous substances?

  • Are containers and secondary containers are properly labeled.

  • Are gasoline/fuel containers used for dispensing contained in the appropriate metal safety can?

  • Other Hazardous Materials issues?

Environmental Controls

  • Is there proper lighting (general and task) for the work areas?

  • Are methods of controlling dust being used?

  • Are workers being advised of hazardous conditions (noise, respiratory, etc.) in nearby work areas.

Ladder Safety

  • Are ladders maintained in good condition?

  • Do extension ladders extend 3 feet the elevated surface and are secured?

  • Are workers demonstrating safe work practices on ladders?

Scaffolding

  • Is scaffolding present on-site?

  • Is the scaffolding being inspected by a competent person on a daily basis when used (green tagged)?

  • Is in the scaffolding being properly maintained with all framework and planks in place?

  • Is there proper access to the scaffolding?

  • Are toeboards being used to prevent falling object hazards?

  • Are workers properly and accessing and using the scaffolding?

Rebar and Concrete Construction

  • Are all impalement hazards corrected?

  • Are concrete and rebar activities taking place on-site?

  • Are exposures to silica dust during cutting, drilling, grinding, coring, etc... being controlled by the use of wet operations or dust collection systems (vacuums) and respiratory protection per OSHA Table 1?

  • Are column and wall forms being erected safely?

  • Are stripping of forms being conducted safety?

Other Silica Activities

  • Are there other silica related activities occurring on site (i.e. Mixing of dry concrete, mortar, joint compound or other silica containing products, Drywall Sanding, Masonry or Stucco, Drilling, Sawing, Coring, Grinding, etc.) ?

  • Are exposures to silica dust being controlled by the use of wet operations or dust collection systems (vacuums) and respiratory protection per OSHA Table 1?

  • Is the proper respiratory protection being used, as required?

  • Are other trades in the area being exposed to dust without controls or protection?

Power and Hand Tool Safety

  • Are tools used by employees in good condition, with guards in place, cords in good condition, etc.?

  • Are workers observed using tools in a correct manner, and according to manufacturer guidelines?

  • Do operators of powder actuated tools have their valid certification card?

  • Is the proper signage used when operating powder actuated tools?

  • Is the proper PPE being worn for the tools being used?

Electrical Safety

  • Are extension cords, "spider boxes" and "baloney cords" in good condition?

  • Are electrical cords protected from damage or exposure to wet conditions?

  • Other Electrical Safety issues?

Hot Work - Welding, Cutting, Brazing

  • Are there activities requiring hot work procedures?

  • Are hot work permits being obtained and procedures followed, including fire watch times?

  • Is fire extinguishing equipment in place?

  • Have all flammable and combustible materials been removed and all floor openings been covered or shielded?

Compressed Air and Gasses

  • Are there compressed gas cylinders on-site?

  • Are compressed gas cylinders in good condition and properly labeled?

  • Are compressed gas cylinders properly secured to prevent tipping, falling or rolling and with the valve covers in place and regulators removed when not in use?

  • Do compressed gas cylinders have proper means of transport?

  • Is firefighting equipment available nearby the gas cylinders?

  • Do one inch or larger compressed air hoses have "whip preventers" or safety chains in the event of separation?

  • Are air hoses in good condition?

Crane and Rigging Operations

  • Are crane operations or lifting of loads being conducted?

  • Are cranes being inspected daily and documented?

  • Are alarms or other means used to announce lifts/picks overhead to warn workers?

  • Is the crane or other lifting device observed to be operated in a safe manner?

  • Are loads properly rigged and secured?

  • Are all straps, chains, hooks, shackles, etc. used in lifting loads in good condition?

  • Are proper hand signals or other means of communication being used during crane operations?

Equipment - Forklifts, Excavators, Front Loaders, etc.

  • Are heavy equipment or forklifts on-site?

  • Are equipment operators properly certified for the equipment they are operating?

  • Are daily equipment inspections being conducted and documented?

  • Are equipment operators observed operating the equipment in a safe manner?

Aerial Equipment and Elevated Work Platforms

  • Are their currently aerial lifts or elevated work platforms being used on site?

  • Are operators properly trained or certified to use the equipment?

  • Are daily inspections being conducted and documented?

  • Are lifts or platforms being operated in a safe manner?

Traffic Control

  • Are there current activities that require traffic control?

  • Is appropriate signage and barricades in place?

  • Are the flagger(s) demonstrating safe work practices and have the correct equipment and PPE?

Excavation and Trenching

  • Are there current activities involving excavation or trenching?

  • Are all excavations over 5 feet in depth properly sloped, benched or shored?

  • Has the competent person completed a documented daily inspection of the excavation/trench PRIOR to persons entering?

  • Is there proper egress available for workers inside of an excavation or trench (max. 25 foot access)?

  • Are all other excavations or trenching properly barricaded or marked?

  • Are spoils properly stored and at least 2 feet from edge of excavation or trench?

Confined Spaces

  • Are there confined spaces on site?

  • Are the confined spaces identified with required signage?

  • Are the confined spaces "Permit Required"?

  • Has the Confined Space Entry Permit been completed and entry authorized by the entry supervisor?

  • If a "Permit Required Confined Space" with hazardous atmospheres, has air monitoring been completed?

  • Is rescue equipment in place during entry operations?

  • Is the confined space entry attendant in place, with no other work activities?

  • Are trained "stand by" personnel with appropriate retrieval and first aid equipment available for any current confined space operations, if required?

Sanitation

  • Are the minimum number of toilets and washing facilities provided?

  • Are all toilets and washing facilities clean and sanitary?

Heat Illness Prevention

  • Is there adequate shade and access to water?

  • Do subs have potable water available for workers?
