  • PRE-SHIFT INSPECTION: Before use each day or at the beginning of each shift, the aerial platform shall be given a visual inspection and functional test including but not limited to the following:

  • Operating and emergency controls

  • Safety devices

  • Personal protective devices, including fall protection

  • Air, hydraulic and fuel systems leaks

  • Cables and wiring harness

  • Loose or missing parts

  • Tires and wheels

  • Placards, warnings, control markings and operating manual(s)

  • Outriggers, stabilizers, extendible axles and other structures

  • Guard rail system

  • Items specified by manufacturer

  • Note anything abnormal or in need of repair:

  • Operator’s Name or I.D. #:

  • Date:

  • Shift:

  • Hr. Meter Reading:

  • Equipment I.D. #:
