1. Working at Heights

Ladders

Scaffolds

  • Unlicensed persons erecting scaffold above 4m?

  • Unsafe or incomplete scaffold?

  • Safe Work Load (SWL) exceeded? (tools, stored materials, number of persons)

Scissor lifts

  • Workers operating lift have been instructed in safe use?

  • Lift is in serviceable condition with log books?

  • SWL information displayed?

Truss and Rigging

  • Unsafe or damaged truss?

  • Loads lifted over persons, close proximity to obstacles incl. overhead power lines?

  • Unsafe or damaged lifting equipment, including ropes, slings, chains, hooks?

  • Lifting equipment unlabelled, does not meet AS, no SWL displayed?

  • SWL of plant or any lifting equipment exceeded?

  • Are safety wires attached to equipment that can fall? Take special care with lights and projectors.

7. Hazardous Manual Tasks

  • Lack of assessment of hazardous manual tasks?

  • Inadequate risk controls? (examples: no job rotation, lighter loads, trolleys, lifting equipment, etc)

  • Lack of training in risk controls and safe lifting/ team lifting techniques?

13. Electrical Hazards

  • Unsafe electrical leads? (damaged, out of test date or untested)

  • Unsafe Appliances (do they have a valid test and tag status?)

  • Lack of RCD protection?

  • Overloading outlets?

  • Electrical equipment near water?

  • Electrical equipment near flammables?

  • Electrical equipment near chemicals that could cause damage?

Equipment

  • Is all equipment fit for the purpose it is being used for?

  • Is their any risk, even if minor of equipment falling from height.

  • Are safety cables fitted to equipment at height?

  • Are drape base plates adequately sand bagged? Can draping be blown over by drafts?

  • Is their any risk of heat from light fixtures causing a fire?

  • Is their any chance of causing hearing damage from the installed sound system?

  • Is equipment mounted on stands safe, balanced and not able to fall over?

  • Are there any trip hazards from: cables, wires, equipment, stands and structures?

9. Emergency Response

  • Lack of, or no emergency plans for site?

  • Emergency procedures not displayed at site?

  • Lack of, or inadequate first aid kits aid equipment for site?

12. Work Environment

  • Lack of/or inadequate amenities? (toilets, wash areas, lunch rooms, etc)

  • Insufficient lighting?

  • Do workers have adequate workspace to preform their duties without risk of injury including computer RSI?

  • Are trestle and equipment tables firm, stable and of suitable strength?

Corrective Actions

  • Enter any corrective actions that will be undertaken

Sign Off

  • On site representative / Technical Director or Technician in charge of job

  • Auditor's signature / Person checking job (can be the same as above)
