NOTICES & SIGNS

  • Are emergency evacuation routes and guidelines posted?

  • Are emergency phone numbers and procedures posted conspicuously?

Are all required notices posted in conspicuous locations for all to see?

GENERAL HOUSEKEEPING

  • During the colder months, are all thermostats throughout the warehouse set at 65 degrees?

  • Are all areas, including storage rooms, kept in a clean and orderly fashion?

  • Are all ceiling tiles in place and in good condition?

  • Please take pictures of damaged ceiling tiles and notate location.

  • Are there any water leaks coming from the roof, skylights, or doors?

  • Are the floors clean, dry, and free of any slip/trip hazards?

  • Stairs and doorways are clear and well-lit?

  • - Are all working surfaces clean and organized?

  • - Are passageways and exits free of storage and obstacles?

  • - Are breakrooms clean and orderly? Are refrigerators, microwaves, eating surfaces clean?

  • -Are restrooms stocked with toilet paper, hand soap, and paper towels?, and are also clean and free of hazards?

  • Are all toilets, urinals, and sinks functioning properly?

  • - Are there enough waste baskets near all workstations and throughout the warehouse?

  • - Are all wastebaskets emptied regularly?

  • Why aren't wastebaskets emptied regularly?

  • - Are pallets that are being used or stored in good condition? (No broken or protruding wood or nails)

  • - Are empty pallets stacked in a safe & orderly manner (not to exceed 6 feet in height)

  • Are spill cleanup absorbents readily available?

  • - Are rodent and other pest control devices in place and in proper working condition?

EMERGENCY EXITS & PATHWAYS

  • - Are emergency exits and emergency evacuation routes of travel unobstructed and clearly identified?

  • - Are aisleways designated, permanently marked, and kept clear to allow unhindered passage?

  • - Are Fire doors NOT blocked, NOT locked, and operate freely?

  • - Are the emergency door alarms working properly? (DETEX Alarms)

  • - Are there illuminated "EXIT" signs next to each emergency exit?

  • - All "EXIT" signs should have a battery backup so that the sign stays illuminated during a power outage. All "EXIT" signs and emergency lighting should be tested regularly by pressing the "Test" button usually located on bottom end of each unit. Are all "EXIT" signs working properly?

WAREHOUSE RACKING & SHELVING

  • - Are storage areas kept free from accumulation of materials that could lead to tripping, fire, explosion, or pest infestations?

  • - Is there a minimum of 18" clearance between the sprinkler heads and the items being stored?

  • - Is there a minimum of 2 feet walkway between all shelving and any other items?

  • - Shelves, racks, and platforms are not overloaded.

  • - Is there garbage or debris on shelving?

  • - Are all items on shelves properly placed, neat & orderly?

  • - Are there items on shelves or pallets that protrude passed the shelf?

  • - Are heavy and awkward sized or shaped items stored on lower shelves low enough to be safely moved when necessary?

  • Are loose or unboxed items stored in a manner that would prevent the stack from creating a falling hazard or causing box crushing (i.e. limiting the height of stacked items or shrink wrapping the boxes together)?

  • - Are racks & shelves damaged, bent, or broken?

  • Take pictures and identify exact location of all damaged shelves, racks, etc

LOADING DOCKS

  • Are all dock doors in good working condition? (Ensure all doors in use open and close smoothly and lock properly.)

  • Are all dock area lights in good working condition?

  • Are gates or chains in place to prevent a person from falling off of the dock?

  • Are bridge plates in good working condition and being used properly when necessary?

FIRE EXTINGUISHERS

  • Are there sufficient number and type of fire extinguishers located throughout the facility?

  • Are fire extinguishers located in easily accessible locations and free from obstructions?

  • Are ALL fire extinguishers securely mounted with the handle at a height of no less than 3 feet and no more than 5 feet?

  • Are all extinguishers filled within the green area of the gauge and have a safety pin in the handle

  • Are there "Fire Extinguisher" signs for each unit, and are these clearly visible?

  • Does each fire extinguisher have a valid certification tag?

ELECTRICAL SAFETY

  • Are high voltage panels in locked closets or in separate rooms and clearly marked as such?

  • Fuse and control boxes are clean and closed?

  • Are all circuit breaker panels properly marked?

  • Are all lights properly mounted, and in good working order (i.e. not burned out, bright light)?

  • Are the areas surrounding electrical panels (at least 36" perimeter) free & clear of all obstructions.

  • Are cover plates in place on all switches, outlets, and junction boxes to eliminate exposed wires?

  • Are extension cords being used temporarily? Note: Extension cords can only be used for temporary power needs. Surge protectors, not power strips are permitted. All electrical cords from appliances should be in good condition and directly plugged into an outlet.

  • Are portable electrical heaters in good condition and unplugged at the end of each day?

  • Are Lockout/ Tagged Out (LO/TO) procedures being followed?

  • Annual infrared inspection of electrical panels completed?

FIRST AID

  • First Aid Kits are adequately sized and located in conspicuous locations throughout the facility with appropriate signage?

  • Complete replacement or replenishment First Aid kits are on-site and readily available?

  • Are first aid kits inspected monthly and are they fully stocked?

  • Do all forklift charging areas contain a full personal protective equipment (PPE) kit?

  • Is there an Emergency Eyewash Station within 50 feet of any charging station?

  • Does the Eye Wash Station have a maintenance tag attached? Has the Eye Wash Station been inspected in the past month?

  • (Make sure to initial & date) the inspection tags monthly and attach picture)

  • Are Eyewash Stations tested and flushed semi-annually (every 6 months) and recorded on the maintenance tags?

  • Is the Eyewash Station full of water?

FORKLIFTS & SAFETY

  • Are all forklift operators certified by MODA?

  • Are all forklift and order picker operators wearing their certification cards while operating these vehicles?

  • Are forklift inspection forms being completed daily and kept in a binder or folder?

  • If forklifts are broken, are they tagged "Out of Service" and being promptly repaired?

LADDERS & SAFETY

  • Are all ladders in use of the appropriate size and type?

  • Do all ladders in use have non-slip surfaces and are being used as intended?

  • Are ladder inspection tags attached to each ladder and being completed every three months?

  • Are ladders inspected at least quarterly?

  • Are portable ladders in good repair and safe to use?

  • If ladders are in fair or poor condition, they should immediately be tagged with an "Out of Service" tag and should not be used until repaired or replaced.

  • Are ladders being stored properly in a manner not to fall, tip, or slip?

MACHINERY & TOOLS

  • After use, are tools returned back and stored in a safe, clean, and organized manner?

  • Are hand tools all in good condition, and taken out of service when appropriate?

  • Are electrical tools well maintained, and in good working order?

  • Are machine and belt guards in place and in good condition?

  • Are carts, storage baskets, rolling racks, etc., well maintained and in good working order? If not, are they tagged "Out of Service"?

EXTERIOR SAFETY

  • Are the grounds free of debris and other obstructions?

  • Walkways, curbs, and parking lots are free of cracks and heaving that might be a slip/trip/fall hazard?

  • Are the exterior lights all in working order? (All sides of the building)

  • Are all walkways, entrances, stairways, and emergency exits clear and unobstructed by snow, ice, water, or debris?

  • Are the dock areas free from obstructions including water, snow, ice, and debris?

  • Is the area around fire exit doors free and clear of obstacles?

  • Is ice melt or sand readily available near entrances and exits in areas that are likely to accumulate snow or ice?
