1. Reservation Call

  • 1.01 Was the phone answered within three rings.

  • 1.02 Were you asked for your phone number, first name and last name.

  • 1.03 Was your information repeated and verified that it was correct.

  • 1.04 Were you asked if you wanted to receive email confirmation of the reservation.

2. Initial Impressions

  • 2.01 Did the outside of the restaurant look warm and inviting?

  • 2.02 Were the chalkboard specials legible and appealing (no smudges)?

  • 2.03 Was the seafood case presentable, clean and had no odor?

  • 2.04 Were the tables and chairs free of crumbs when you sat down?

  • 2.05 Was the floor free of debris?

  • 2.05 Were all light bulbs illuminated?

  • 2.06 On a scale of 1-5, how welcome did you feel the moment you arrived in the restaurant.

3. Bar Service

  • 3.01 Was the bar clear of debris and wiped clean?

  • 3.02 Was the bar set up and ready for service (no clutter on bar from setting up)?

  • 3.03 Were the bottles dusty?

  • 3.04 Were the televisions on and showing sports events, special events or news?

  • 3.05 Were you offered a specific drink?

  • 3.06 On a scale of 1-5, how welcome did you feel at the bar.

4. Service with a Smile

  • 4.01 Were you greeted at the door with a smile and eye contact?

  • 4.02 If there were tables available were you seated immediately?

  • 4.03 Did the host(ess) use your name when seating you at your table?

  • 4.04 Once at your table, were you greeted by your server within 2 minutes?

  • 4.05 Did your server point out specialty cocktails and wine list on the back of the menu?

  • 4.06 Did your server make personal recommendations or offer something specific to drink?

  • 4.07 Did your server tell you about the specials?

  • 4.08 Were you offered the correct, most up to date menu?

  • 4.09 Were menus free of stuck on debris and/or markings and wrinkles/tears?

  • 4.10 Was server knowledgeable about menu, specials and wine without reading them?

  • 4.11 Were any menu items out of stock (excluding specials)?

  • 4.12 Did the team member offer you any additional items to compliment your meal?

5. Delicious Drinks

  • 5.01 Did your drinks arrive in a reasonable amount of time after you ordered them?

  • 5.02 Were drinks kept 2/3 full from beginning to end (water, iced tea, soda)?

6. Fantastic Food

  • 6.01 Was the lady's order taken first?

  • 6.02 Was each order repeated/clarified back to each guest?

  • 6.03 Was silverware on the table by the time food was on the table?

  • 6.04 Did app plates and/or oyster set ups arrive before or at the same time as the apps?

  • 6.05 If salad and app ordered, did server ask if you wanted them together or coursed separately?

  • 6.06 Did apps/salads arrive within 10 minutes of order?

  • 6.07 Were apps and salads cleared as finished?

  • 6.08 Were entrees delivered within 10 minutes of apps being cleared?

  • 6.09 If NO apps ordered, were entrees delivered within 25 minutes of order?

  • 6.10 Did food come out as ordered?

  • 6.11 Were the plates wiped clean with no food or fingerprints around the edges?

  • 6.12 Was hot food served hot and cold food served cold?

  • 6.13 Was the food seasoned well (too salty, not salty enough, etc.)?

  • 6.14 On a scale of 1-5, how would you rate the overall quality of entrees?

  • 6.15 Did your server check back within 3-4 minutes of entrees arriving?

  • 6.16 Did a manager visit your table after entrees were delivered?

  • 6.17 Were entrees cleared within a reasonable amount of time?

  • 6.18 Was dessert offered to the table?

  • 6.19 Were desserts requiring prep time offered before entrees finished i.e. bread pudding?

7. Table Maintenance

  • 7.01 Were tables pre-bussed during visit and wrappers/small papers removed?

  • 7.02 Were buckets on table clean, free of debris, free of sauce splatter?

  • 7.03 Did buckets contain Datil sauces, tobasco, ketchup, Datil vinegar, extra napkins?

  • 7.04 Were the condiments in the buckets at least half full?

  • 7.05 Were dirty tables in surrounding areas cleaned in reasonable amount of time?

8. Sparkling Restrooms

  • 8.01 Were toilets clean?

  • 8.02 Were soap, toilet paper and paper towels stocked?

  • 8.03 Were there any foul odors in the restroom?

  • 8.04 Were the restroom floors clean and free of debris?

9. Payment

  • 9.01 Was the check presented in a timely manner?

  • 9.02 Was payment processed in a timely manner?

  • 9.03 Was marketing materials for promos offered with check presenter?

  • 9.04 Was the check presenter clean and free of food debris, sticky residue?

  • 9.05 If paid with credit card, was your name used?

  • 9.06 On a scale of 1-5, was the cost of your meal equal to the value of your experience

10. Lasting Impressions

  • 10.01 Was your server well groomed: clean shaven (beard neatly trimmed), clean apron, ironed shirt, long hair pulled up, shorts appropriate length, fingernail polish not chipping, no facial jewelry?

  • 10.02 Was the manager visible in the restaurant?

  • 10.03 Did staff yield right of way to you when moving through the restaurant?

  • 10.04 Were you sincerely thanked and invited to come back by anyone (host(ess), server, bartender)?

  • 10.05 On a scale of 1-5, what was the overall professionalism of all staff on duty

  • 10.06 Would you return to this restaurant on your own time?

  • 10.07 Did any manager, server, bartender, host(ess) do anything special that stood out (please use names if possible)?
