What activity was being observed at the time of the audit? Describe any unsafe acts.

1. stop and observe (looking for safe and unsafe behaviour)

2. put the person at ease (Start the conversation in a positive way)

3. Explain what you are doing and why (I’m doing a 9 step audit to improve safety)

4. Ask what the job is and the stages in it (an open question so the observed tells you what they are doing)

5. Praise aspects of safe behaviour (give recognition for the work or appearance of the site)

6. Ask them what is the worst accident that could happen and how (get the observed to picture the worst accident. Don’t jump, let them think)

7. ask why the unsafe behaviour happens (try to find out how and why the worst accident could happen)

8. Ask what corrective actions are required (let the observes come up with the solution so they take the ownership)

9. Get commitment to act (so they will take that critical step in the process which makes all the difference)

By this stage you should have finished your conversation with the observed. Do you feel that you understood the job and the stages in it? Did the observed tell you what the worst accident could be and how this could happen? Did they identify the corrective action to take and commit themselves to act? If so then you have successfully completed the audit.
