  • Are you having close interactions between groups of workers?

  • Are these group interactions necessary for work?

  • Please record the names of your work colleagues you are working closely with today?

  • Have any customers been on your premises today?

  • Did you have any physical contact with these customers?

  • Did you go to a customer's site / residence for work today?
