Audit Protocol

1. Quarrying shall follow an agreed design.

2. Site rules shall be developed, documented and well communicated to all workers. Rules include access to restricted areas (active faces, toes and crests of highwalls, stockpiles, tips etc.); parking and approaching mobile equipment on roads and on benches; speed limits etc.

3. Clear signage, especially directional signage.

4. All heavy mobile equipment required to operate on benches and under highwalls or excavated stockpiles where there is a potential for a rollover or engulfment by material shall be fitted with ROPs and FOPs.

5. All light vehicles and mobile equipment shall be fitted with strobe lights or flashing lights.

6. All light vehicles entering operational areas shall be fitted with a buggy whip with red flag at least 3 m from the ground.

7. All roadside edges that have a drop off shall be protected by berms (windrows) of at least half the height of the largest mobile equipment wheel.

8. No nighttime operations shall be permitted unless approved by the Department Manager and lighting stations are in place.

9. Rules or work instructions shall be documented for use of explosives (Approaches when loading / firing; handling misfires, signage; distance maintained away from faces).

10. All personnel on the site shall be fully trained and certified by the Quarry Manager as competent to work on the site (and for the tasks they perform) or are fully escorted by a competent person. This requirement must be included in the induction and can be reinforced with signage at the quarry entrance (E.g., "STOP: Do not pass this sign unless you are certified by the Quarry Manager or are escorted")

11. Roads shall be constructed so that one way roads are 2.5 x width of the largest vehicle and two way roads are 3.5 x width of the largest vehicle.

12. Speed limits shall be determined and defined by a specific quarry only risk assessment, communicated, signposted and reinforced.

13. Maximum road inclinations shall be 10%

14. A rule that prevents people from coming within 2 m of a bench or mine edge shall be enforced. Within 2 m, barriers or harnesses shall be used to protect from falls.

15. Quarry light vehicles (<4.5 T) shall be made highly visible by having: -a safety flag/whip with the flag at least 3 m above ground level. -a revolving flashing light on the roof and reflective tape around the vehicle. -Vehicles not meeting these requirements are not permitted to enter the quarrying operation.

16. Signs to this effect (ref. item 15) shall be installed at the start of all access ways to the quarrying operation.

17. Where practical, physical barriers shall be installed to stop external vehicles from accessing the quarry area.

18. Quarry access shall be limited to staff, contractors and escorted visitors. If it is not possible to isolate the quarry from locals, dual language signs shall be installed warning of the hazards. A risk assessment considering all options (Protected pathways; alternative routes, school education etc.) shall be conducted.

19. Right of way (vehicle priority) rules shall be established across the quarry site and haul roads and are communicated to all staff and contractors through training.

20. A procedure for mobile equipment electrical and mechanical isolation shall be place for all vehicles.

21. All mobile equipment shall have operational reversing alarms installed.

22. In case of heavy vehicle breakdown, emergency ramps or other emergency stops are designed and installed.

23. Blasting procedures shall be documented and communicated and readily available. They include treatment of misfires, waste disposal, inventory management and secondary blasting.

24. Only Non Electrical NONEL accessories are allowed for blasting, including initiation unless they are not available and then electrical initiation systems may be used if approved by the Site Manager.

25. Blasters and helpers have been certified (Government certified where applicable) for explosive handling / use.

26. Blasting signage (date & time) shall be located at the quarry access / gates and updated prior to each blast.

27. A secure place (shelter) for the blasting master to be protected from fly rocks shall be available.

28. Specific risk assessments hall be conducted for each secondary blasting session (must include an increased evacuation radius). NONEL explosives only are used.

29. Magazines shall be secure and free of vegetation that poses a fire risk.
