  • Directions for the Auditor:
    - Prior to getting on site ensure you have approval from the Operator, Team Leader and customer site rep, to conduct the audit.

    - When on site you will l need to: stay for about 2 hours to see most activities, observe manual handling techniques, watch the MMU manoeuvring; inspect the MMU; ask the operator some questions; watch hose handling.

    - Conformance checklist and points scoring.
    UN - Unacceptable (0), NI - Needs Improvement (1/2), ACC - Acceptable (1), NA 0 Not Applicable

    - Tick the correct box when finished, then pass this audit to the plant supervisor for review.

PPE

  • The operator must be wearing a hard hat, ankle high lace up safety boots, safety glasses, hearing protection, and gloves.

MMU CONDITION

  • Prestart inspection completed?

  • First aid kit onboard MMU and appropriately stocked.

  • Sealed eye wash bottles (3x) are easily accessible and stored on the MMU.

  • Hearing protection located on MMU?

  • Dust protection located on MMU?

  • Chemically impervious gloves stored on MMU?

  • Up-to-date EPG's stored in the driver side door of MMU?

  • Confined spaces labelled with confined clearance stickers?

  • All machine guard and inspection points hae an isolate before opening sticker?

Access/Egress and Bench Manoeuvres

  • Has positive communication been made with active mining circuits when travelling between the bench and explosive reload?

  • MMU operator checks that access onto the bench is safe and adequate?

  • Take a photo of access onto the blast bench.

  • Operator uses a spotter when reversing or performing challenging manoeuvres?

  • Where can the TMP be found and has the operator signed onto it for the day?

MMU Operations Personnel

  • When parking the MMU the operator parks in a manner so that if the brakes fail it will not roll towards the pit edge?

  • When entering and exiting the cab of the MMU, the employee maintains 3 points of contact?

  • All waste/spillage is disposed of properly?

  • On-bench loading completed safely and spills rectified?

  • When can the operator work on top of the MMU and what is required when doing so?

Hose Handling

  • Manual handling with the 1" hose required on this day?

  • Has the operator warmed up and stretched prior to commencing manual handling tasks?

  • The operator maintains good posture at all times when manual handling and if required uses various methods of hose handling?

  • When pulling the hose the operator holds the hose close to the hips with both hands and drives forward with the legs?

  • The operator faces the pit edge when working within 3 meters of it?

General Safety Questions

  • Is smoking allowed on this particualr site?

  • How far away from explosive products must you be to have a cigarette per Orica standards?

  • Name one of the principle hazards associated with this site?

  • Name some hazards you would encounter when driving onsite?

  • How often should the MMU be calibrated?

  • Is the product pump within the calibration timeframe?

  • How often should you perform quality checks on the product being loaded?

  • If a problem is discovered what would be the necessary escalation steps?

  • Why is it important to record the amount of product going into each blast hole?
