General Job Site Safety

  • Lack of/or inadequate amenities? (port-o-johns, wash area, break area, etc.)

  • Safe access into and out of building or building pad?

  • Firm, graded, level ground surface for crane operations?

  • Housekeeping?

  • Fuel tanks are located in spill proof containers, that can contain the full contents of the tank?

  • A 20 lbs. fire extinguisher is located at least 20 feet and no more than 75 feet from the fueling area? (the fueling area is where fuel is stored and anywhere equipment is fueled)

  • Power lines are. Ore than 20 feet away from the point of any work to be performed? (applies to cranes, forklifts, aerial lifts, etc.)

  • All moving equipment is traveling at safe speeds? (no faster than a brisk walk)

  • No work is performed with wind speeds above 28 m.p.h.?

  • Adequate drinking water is supplied for workers?

  • Water coolers are sealed and dated with today's date, and clean cups are provided?

Personal Protective Equipment

  • Sturdy work boots or steel toe boots are worn? (as required by project)

  • Long pants are worn? (no shorts)

  • Shirts with a minimum 4 inch sleeve?

  • Gloves are worn at all times when handling steel?

  • Safety glasses meeting at a minimum ANSI standard Z87.1 are worn? (Z87.1 must be stamped or printed on the lenses or frames)

  • Dark tinted safety glasses are only worn outside?

  • Hard hats meeting at a minimum ANSI standard Z89.1-1971 are worn?

  • All welders wear a protective welding hood while welding? (hard hat must be worn under welding hood at all times)

  • Workers grinding, cutting, or performing any work that does, or may produce flying objects and particles are wearing safety glasses and a face shield?

Fall Protection

  • Fall exposures of 6 feet or more? (including pits and trenches)

  • Is suspended working platform being used correctly?

  • 100% tie off in aerial lifts at all elevations?

  • Use of correct anchor point in aerial lifts?

  • Swing fall exposure?

  • Personal fall arrest equipment adjusted and worn properly? (chest strap, leg straps, d ring located between shoulder blades)

  • Retractables connected to adequate anchor points?

  • Perimeter (cable) guardrails installed and adjusted properly? (must be a minimum of 39" above walking surface with 200 lbs. of downward force applied)

Steel Erection

  • All columns are supported by a minimum of four anchor rods, tightened prior to the release of hoisting lines?

  • Structural stability is maintained at all times? (columns are braced in a minimum of two directions within a reasonable time)

  • Minimum of two bolts per connection during erection?

  • All bolts are installed and torqued prior to loads being applied to the structure? (mezzanine pours, rtu placement, etc.)

  • Joists are connected at both ends prior to releasing hoisting line?

  • Joist Panels are connected at all four corners before releasing hoisting lines?

  • Minimum of one row of bridging is installed prior to placement of deck bundles?

  • Where X bridging is required it is installed between joists prior to release of hoisting lines?

  • No personnel are on top of joists without all bridging installed?

  • Deck bundles placed on a minimum of three joists?

  • Deck bundles placed within one foot of the bearing end of the joist?

Crane and Hoisting Operations

  • Daily inspection documented?

  • All safety devices on the crane are operational?

  • Swing radius barricaded?

  • Crane located on a firm level surface?

  • Routes of suspended loads do not pass over workers?

  • Are whistles being used to warn others of a suspended load?

  • Appropriate rigging specifically designed for multi lifts is used when hoisting two to five members at once?

  • No more than five members hoisted together in a multi lift?

  • Members lifted in a multi lift are a minimum of 7 feet apart?

  • All rigging from a forklift is performed with a hook or hoisting attachment? (no rigging on the forks)

  • Forklift operator is wearing a seat belt?

  • Forklift loads are kept close to the ground when traveling?

  • Forks are placed on the ground when forklift is stopped?

  • Operator of hoisting equipment does not leave the operators position while a load is suspended?

  • Lifting capacity is clearly displayed on all hoisting attachments?

  • Tags are installed on all rigging equipment, and are readable?

Decking and Controlled Decking Zone

  • Perimeter warning lines erected? (15 feet from edge, line must have a breaking strength of 200 lbs.)

  • All workers in CDZ tied off 100%?

  • Retractables connected to adequate anchor points?

  • Safety deck attachments (at least two) only in the CDZ? (final connections performed in the protected area outside the CDZ)

  • Warning lines are between 39 and 45 inches above working surface?

  • Loose metal decking is secured against accidental displacement at the end of the shift?

  • Area below decking operations is barricaded, or warning signs and a safety monitor are present?

Ladders

  • Ladder type is correct for the task?

  • Extension ladder extends a minimum of 3 feet above upper surface?

  • Fall protection equipment is used when working from a ladder

  • Step ladders are not used to access upper levels?

  • No aluminum ladders are used?

Welding, tools and elctrical safety

  • No damage or repairs in welding lead 10' from stinger?

  • Are welding leads in good working condition?

  • Welding ground cable connected?

  • Location of welding leads and extension cables are safe from accidental displacement or damage by personnel or equipment?

  • GFCIs are being used?

  • Extension cords have no repairs in the cable, and ground prong is present?

  • Extension cords are a minimum of 12 gauge wire?

  • All guards are in place on tools?

  • Electrical cords of all tools are serviceable and have no repairs or missing ground prongs?

  • Are the gages working properly when installed on oxygen/acetylene bottles?

  • Are lines free of any damage?

  • Are flash arrestors being used correctly?

Fire prevention

  • Has work area been inspected?

  • Is work area free of any combustibles?

  • Are there any other trades in the area? Have they been notified?

  • A 20 lbs. fire extinguisher is located at least 20 feet from any open flames?

  • Is fire extinguisher within 15 feet of reach of fire watch?

  • Is fire blanket being used properly?
