Pre-Work Paperwork

PPE / Equipment

  • Is there a Phase Check availabe and is it in good order?

  • Does the vehicle have a first aid kit and Fire Extinguisher available?

  • Does the operative have the correct Arc PPE and is it for for purpose?

Work Process

  • Has the operative parked correctly at the work location?

  • Has the operative set up a safe work area around the PL column or LA Pillar?

  • Has the electrian assesed the PL or LA pillar using the Phase check?

  • Is the G.O aware of the dangers of accessing a live source and is he standing a<br>safe distance away while the PL or LA door is being removed?

  • Is the electrican using the corrected insulated tools?

  • Has the operative removed the PL column or LA pillar door correctly applying<br>safe practice?

  • Has the operative completed the visual survey without risk?

  • Once the visual survey was complete did the operative reattcahed the PL or LA<br>pillar door safely?

  • Has housekeeping been maintained throughout the work process?

Live Works Checklist

  • Is the PICW identified and present on site?

  • Are all persons working on site approved PL Opening Column Doors

  • Is the full range of approved PPE in use and in good condition? Helmet with full face visor, High Vis Vest, Rubber-soled Safety Boots, Arc Protective Overalls?

  • Is the full face visor fully engaged and upper body fully covered when making/breaking connections or where there may be danger of flashover?

  • Do the approved insulated protective overalls carry the “double triangle” 1000V stamp?

  • Are the Gloves checked before use? a) Stretched and b) Close-up Air Test

  • Are approved Live Work Tools and Equipment stored separately from other general tools etc.?

  • Are the live work tools and equipment in good condition? e.g. not soiled etc.

  • Has the contractor received permission from ESB to energise the PL switchwire?

Hammer Test

  • Is area clear to do hammer test?

  • Access the pole for any cables before hammer test?

  • If cables are identified, is test been carried out on side of pole without cables?

  • Is safety glasses/ visor worn at time of hammer test?

  • Is G/O stopping any pedestrians while the hammer test is been carried out?

  • If pole is defective, has it been recorded?

  • Is house keeping been maintained after hammer test is carried out?

Working at height

  • Hoist in use?

Hoist Parking & Set-Up

  • Are appropriate traffic management controls (signage, cones, barriers) and CSCS approved persons in place?

  • Are all hazard warning lights & flashing beacons working and in use?

  • Are all host operatives fully qualified?

  • Are vehicle jacks fully extended on all sides and deployed on level surface with pressure pads, where required?

  • Is vehicle correctly positioned for incline with front wheels angled into kerb, wheel chocks deployed and handbrake fully engaged?

  • Inspect copy of Manufacturer’s Certification of vehicle for Class C operation ( 46kV) and cross check with vehicle name plate

  • Is fully body harness with fall-arrest attachment in use and attached to bucket anchor point?
