  • Does site comply with national and local governing laws and regulations and contract language as written?

  • Have the sites vehicles received regular maintenance inspections and, are they documented per local and contract requirements? (ex. Fluids,tire pressure, lights, horns, etc.)

  • Do operators of casual vehicles and PIT's hold a current and relevant permit or license?

  • Are PIT's and casual vehicles equipped w/ seat belts, a horn, back-up warning devices, rollover protection (for fork trucks), protection from falling objects, and directional lighting?

  • Are powered industrial trucks modified in any way that would affect warranty or safety devices w/o manufacturer approval?

  • Are powered industrial trucks labeled with the model, serial number, approximate weight, SWL, and certification by the manufacturer. All should meet local and national standards, labels should be legible. Attachments should be identified and labeled with capacity and weight.

  • Observed all powered industrial trucks were visually inspected prior to use (prior to each shift) by the operator.

  • Site has process for identifying vehicles that are not safe for operation (red tag or other process) and such vehicles w/ defects are reported immediately.

  • Does site have available, qualified instructors for PIT safety training? (qualification through an outside training provider or internal "train the trainer" program).

  • Do all operators receive formal training within applicable local and OSHA training requirements including, a hands on evaluation?

  • Has site management received basic PIT operation safety training or equivelant?

  • Observe gantry crane and/or transtainer operators for safe lowering of a load onto a PIT in order to prevent jostling of the PIT operator in the cab as well as, raising and lowering the spreader slowly after landing the load.

  • Does facility enforce policy prohibiting use of cell phones, radios, personal electronic devices while equipment is in motion? This includes all employees; transient and permanent.

  • Does site enforce safe operating rules as established by labor contract or site specific rules and, how are they enforced?
