Is the traffic control contractor controlled?

Adequate quantity of signs and cones?

Adequate quantity of flaggers and pilot cars?

Are PRSI workers safe within the traffic control zone?

Are prime contractor workers safe within the traffic control zone?

Are pedestrians and the traveling public safe within the traffic control zone?

Does traffic volume or speed require any additional traffic control devices?

Are there any blind spots that need to be corrected or improved?

Is the PPE being used in accordance with company policy?

High visibility vests?

Hard Hats?

Safety Glasses?

Safety boots?

Hearing Protection?

Long sleeves and pants?

Are visitors wearing appropriate PPE and following safe work practices?

Are water coolers available and clean?

Has all of the equipment inspected and is in proper working condition?

Is the equipment outfitted with the appropriate guards and are they in working condition?

Have all visible leaks on the equipment been repaired?

Does all equipment have functioning and non-expired fire extinguishers?

Does the equipment have shade canopies and are they in working order?

Does the equipment, if equipped have working backup alarms?

Are Lock Out/Tag Out procedures being followed while working on equipment?

Are the headsets functioning properly and is there good communication between the operators?

Are oil/fuel tanks properly protected and is containment in place in the event of a spill?

Are hot work procedures being followed?

Does the foreman and/or lead man know where the closest urgent care or hospital is located?

Are first aid kits easily accessible and fully stocked with supplies?

Are the Safety Data Sheets (SDS) available and does every employee know where they are located?

Have the job tasks (JHA/AHA) for the day been identified and discussed?

Have the underground utilities been located and properly marked?

Have overhead power lines and obstructions been identified and are appropriate warning signs in place?

Have the grade obstructions (manholes, valve cans, survey monuments, etc.) been identified and lowered?

Have all safe guards been taken to deal with current or impending weather conditions (heat, cold, dust, wind, etc.)?

Have pedestrian hazards been identified and resolved?

Are employee vehicles parked in a safe and approved staging area?
