Facility/Plant Inspection

  • Describe incidents or accidents that have recently happened

  • OSHA poster, posted in a common area where it can be seen by all employees

  • Facility free from hazards that could lead to an injury or accident

  • Facility congested, blocked emergency exits, or poor housekeeping

  • Cell phone use, ensure that it does not create a hazard

  • Facility Security: Fencing/Gates, are they working as designed

  • Walkways/Floors/Pavement: free from slip/trip/fall hazards

  • Material Safety Data Sheets (MSDS'): available for employees to review, employees know the location of the MSDS'

  • Extension/Power cords: in good repair, extension cords are only being used in a temporary manner

  • Electrical switchgear & control equipment: free from hazards, kept clear at least 3 feet

  • Equipment / Conveyor(s) equipped with startup warnings

  • Facility equipped with Water & Toilet Facilities

  • Mezzanines: guarded against fall hazards (guardrails, other fall protection devices in use)

  • Stockpiles sloped adequately to prevent unexpected material movement

  • Oil/Fuel tanks (including gas cans): properly protected, secured and no evidence of leaks, fire hazards in the area where these oil/fuels are dispensed are being controlled or eliminated

  • Hot work procedures: being followed when welding & cutting around combustible materials

  • Ladders: in safe working condition, used properly, and stored in a safe manner

  • Emergency action plans: posted at various locations throughout the facility, employees are knowledgeable about the emergency actions they must perform in the event of an emergency

  • Motor vehicles & yard equipment: maintained in good condition

  • Lock Out/Tag Out procedures: being followed while performing maintenance & service of equipment

  • Break areas/lunchroom: clean and sanitary, and are equipped with hand washing facilities

  • Welding & cutting equipment: in good working order

  • Storm water controls: in place to prevent possible contamination from leaks/spills or other sources

  • Dust controls: in use as needed

  • Equipment guarding: in place to prevent employees from coming in contact with the point of operation of the machine (can a person reach around, under, through, or over guarding?)
