  • Briefly outline the requirements of the Mines Regulations 2014 in relation to safe shaft operations<br>*Note: (requires that the shaft and winding systems be assessed, installed to a good standard, and maintained to those standards. Those that use the systems must be trained and competent. There must be suitable signals and communications provided, provision of emergency winding equipment)

S4 Basic Requirements for Safe Operation – the candidate can:

  • Demonstrate an overall understanding of how the system operates<br>*Note: (this includes an overview of all hoists that make up the system, to include; hoist/winch name and purpose, where each hoist/winch is controlled from, ropes and conveyance guidance)

  • Demonstrate how to use the fixed communication systems <br>*Note: (Photograph Shaft signalling system, FEMCO, radio)

  • Demonstrate how the winder/ hoist signalling system works<br>*Note: (Photograph using Shaft signalling system. The operator can identify the signals and the bells)

  • Define the signals that are used for the different operations <br>*Note: (within the response we are looking for an understanding that they are aware of different signals for different operations and different sounds and colours for hoist/winches) [specifics of the signalling system will be covered in the relevant sections of manriding etc.)

  • Articulate the actions to be taken in the event of an incomplete or unclear signal<br>*Note: (If signal is unclear, do not move - until signal is verified, Signa; 1 & ask for signal repeat, follow golden rules for shaft signals)

  • Demonstrate the means for selecting direction of the Hoist or Winch<br>*Note: (Photograph using winder direction control)

  • The winder/ hoist operator can briefly describe the operation of the various safety circuits<br>*Note: (Types of faults that can occur, ie Cat 1, Cat 2 , Cat 3 trips and examples of what may cause these, how are these recorded on the advisory screen)

  • The winder and hoist operator can briefly outline how they would resolve any issues associated with the operation of a safety circuit<br>*Note: (Stop winder, keep stood, report to shift manager and/or Shift Mech/Elect eng, record in log, communicate with shaft team)

  • The winder can describe how to initiate an emergency stop on the winder, and knows the impact this has on the winder/ hoist system<br>*Note: (Describe position of E/stops, and impact on winder of application of E/stop, Awareness of controlled stop being preferred as Estop has potential to cause injury due to the rapid application of breaks – also knows even with this in mind not to hesitate with an Estop if appropriate)

  • The winder/ hoist operator can demonstrate how to recover the winder/ hoist following an emergency stop trip<br>*Note: (Investigate cause, appropriate persons to initiate removal if safe to do so, before winder driver can re-set, any checks to do before re-commencing winding)

S5 Safely Setting Up and Closing Down the Winder/ Hoist

  • Demonstrate the set-up procedure for the winder/ hoist<br>*Note: (Pre-checks on Winder House, Fire equipment in place, guarding in position, general conditions, condensation, signals and comms, HMI working, Check previous reports, handover complete)

  • Describe the action required if the Winch/Hoist does not set up correctly – defects identified)<br>*Note: (Inform relevant supervisor, if safety critical do not operate winch/hoist, record on winder log)

  • Interpret the information on the human machine interface units<br>*Note: (Winder to describe the information on the HMI – including, speed, direction, depth, brake positions, alarms, amps)

  • Articulate the Pre-start tests / Safety system checks that must be completed as part of starting the winder/ hoist or after a prolonged period of shut down<br>*Note: (Checks to be done prior to re-starting, demonstrate a brake test, recording of this, Winder Drivers log Appendix 2)

  • The winder/ hoist driver can show you where the information should be recorded<br>*Note: (Hand over report- winder drivers log – maintenance log)

  • The winder/ hoist operator can demonstrate the procedure for shutting the winder or hoist down<br>*Note: (How to leave the winder if required to shut down, where to leave the conveyance, isolation of the equipment, securing the cabin and building, checks before leaving, securing keys if required)

  • The winder/ hoist operator can outline the tests that must be completed after the shutdown of the system and where the information should be recorded<br>*Note: (Winder drivers log, Confirm isolation of controls)

  • The winder/ hoist operator can verbally outline what should be provide in a post shift verbal handover<br>*Note: (Include – any trips that have occurred, ongoing work in the shaft, any speed restrictions, changes to operations, any heavy or awkward loads transported, significant info, Winder drivers Log Appendix 3 & signed off)

S7 Required Maintenance Tasks

  • Outline that they understand the required maintenance tasks that they must undertake on the system

  • Demonstrate carrying out any pre-use checks / inspections, identifying what they are looking for and why it is important to the safe operation of the hoisting systems<br>*Note: (Winder Drivers log Appendix 1 & signed-off. Support the answer with the photograph)

  • Demonstrate where a record of their maintenance tasks is to be logged <br>*Note: (Photograph)

  • Articulate the actions that they should take if they discover a defect during their maintenance tasks or during the normal operations <br>*Note: (identify steps to remedy, check the steps are taken if appropriate)

  • Articulate the actions that they would take if the defect was such that it could impact the safe operation of the shaft <br>*Note: (As above report submitted to the SS when remedied and safe to do so will sign off the inspection allowing sinking cycle to continue)

  • Can demonstrate they know the reporting lines to escalate a defect <br>*Note: (looking for the candidate to talk about escalation through the supervision and management structure)

  • Articulate how slack rope within the system would be identified and the actions required following a slack rope trip or defect<br>*Note: Encoders located on hoist / headframe drums - The encoder values calculated in hoist and SHEP show a significant difference in depth value will cause an alarm and the safety relay gets activated

S9 Emergency Procedures

  • Articulate the hazards associated with working around the winder/ hoists<br>*Note: (Slips, trips, and falls, contact with machinery, heat and friction, stored energy, electrical etc)

  • Articulate the hazards associated with working around the shafts <br>*Note: (Slips, trips and falls, fall from height, dropped objects, stacked materials, vehicles and plan inadvertently entering the shaft)

  • Articulate how and to whom he would escalate any emergency or unplanned event that has occurred within the shaft. <br>*Note: (Raise alarm. Report immediately to Shift Manager and / or on-shift Mech & Elec Engineers. Inform shaft team)

  • Describe what actions must be followed in the event of a shaft emergency evacuation. <br>*Note: Bio Site Station (Call point), notification of SS, Don escape sets (if needed), extinguish small fires if safe, evacuate shaft, last man out isolation)

  • Demonstrate how they would communicate to any person within the shaft environment <br>*Note: (Photograph operation of radio/femco/tannoy)

  • Demonstrate how they would communicate to any person within the confines of the conveyance <br>*Note: (Photograph use of radio)

  • Describe what actions must be followed in the event that the shaft becomes unavailable for any reason whatsoever. <br>*Note: (Communicate with Shaft team and any persons in conveyance. Evacuate if necessary. Shaft inspection prior to re-entry after permission from Shift Manager)

  • Describe the procedure for establishing the safe use of the emergency winding/ ancillary winding equipment (In the event of an emergency in the shaft<br>Note:<br>*Auxiliary Conveyance is unavailable, then secondary means of egress is via the Muck Conveyance. <br>*The muck conveyance can be accessed through gates on WD5 and WD12 in the event of an emergency. <br>*The full extent of shaft emergency response procedures will be covered in Triton Emergency Response Plan.

  • Describe the circumstances where it may be necessary to establish the secondary / emergency winding systems<br>*Note: (Loss of power, mechanical / electrical failure, damage to rope / suspension equipment)

  • Describe the process in the event of any emergency or unplanned event that has occurred within or around the shaft<br>*Note: Incident Occurs on site – Make safe, Raise the alarm (Cic1). Inform Shift Manager. Remain available for next instruction

  • Identify the location of the firefighting provisions and can articulate the procedure for dealing with a fire in its early stages <br>*Note: (Red Bio Site Station (Call point) Sounds alarm, Combat fire if trained and competent to do so and your safe exit is not impeded, if the extinguisher has no effect evacuate and muster, if possible provide information to Triton

  • Describe the process and location of equipment to evacuate a colleague with major injuries. <br>*Note: Portable extinguishers / Auto suppression system. (CIC 1 or green MAC to raise alarm, establish contact with Triton to provide details, inform SS, notify first aiders, make available any equipment maybe necessary, hoist at slow speed)
