Fire Prevention

  • Are fire extinguishers available and inspected monthly?

  • Fuel Containers are OSHA compliant.

  • Fueling areas are contained and barricade where required.

  • Spill Kits on site.

  • No smoking areas understood by team.

Electrical(GFCI)

  • Ground Fault Circuit Interrupters are being used and have been checked.

  • Electrical cords have protected from pinch points and sharp edges.

  • Damaged Cords have been taken out of service.

Hand, Power, and Powder Actuated Tools

  • The right tool are being used for the job.<br>

  • Guards are in place as manufacture requires.

  • Hand tools have been inspected and removed if damaged.

Fall Protection

  • A fall protection system is in place for fall hazards of 6" or greater.

  • Railing systems; Top-rail 39-45" high, Mid rail 1/2 way between top rail & toe board.

  • Each rail supports: Top-200 lbs. Mid-150 lbs. Toe-50 lbs. of outward force.

  • Personal Fall Arrest Systems (PFAS) are being worn properly.

  • PFAS is connected to a shock absorbing or Self Retracting Lanyard.

  • Lanyard is connected to a 5,000 lb. anchor point.

  • Warning barriers, fencing or flagging are maintained 10' from hazard.

Ladders & Stairs

  • Ladders extend 36" above landing and have been secured.

  • Broken ladders are removed from service.

  • Stepladders when used are fully opened.

  • Stairs with 4 or more risers are equipped with handrails.

Crane Operations

  • Swing radius is barricaded. No one is allowed under suspended loads.

  • Workers understand signals being used. Hand signal chart is on crane.

  • Tag lines are available.

  • A pre-lift meeting and/or critical lift plan was accomplished.

Excavation & Trenches

  • Soil type has been determined.

  • A competent person has inspected the excavation before entry.

  • Protective system used at 5 feet; slope/bench/trench box or shoring.

  • Excavations 4' or deeper have had atmosphere verified.

  • Spoils are two feet from edge. Drainage is away from excavation.

  • Equipment is a safe distance from edge of trench or excavation.

  • Access/egress is within 25' of every worker in trench or excavation.

  • Underground utility installations have been located and hand dug.

  • Employees are wearing high visibility clothing or vest.

Material Handling

  • Materials are properly stored and handled.

  • Good lifting methods are being used.

  • No material or equipment is near or under power lines.

  • Rigging equipment has been inspected. (Tags are affixed)

  • Forklift operators are licensed.

Welding, Cutting and Burning - Hot Work

  • Compressed gas cylinders are upright and secured. (No free standing)

  • Hoses, regulators and back flow are in good condition.

  • Proper separation between fuel and oxygen cylinders. (20' or barrier)

  • Proper PPE is being used. (welding shield-goggle-glasses, FR, respirator)

  • Hot Work Permit has been obtained. (Fire extinguishers are available.)

  • Welding leads are undamaged. (No electrical tape fixes)

Personal Protective Equipment - PPE

  • Hard hats, safety glasses, boots and high visibility are being worn.

  • Hearing protection being used when required.

  • Gloves are being worn when needed.

  • Respirators are used when required.

  • 4 Gas monitor used when and where required. (Monitor Bump tested daily)

Vehicles & Equipment

  • Obstructed view when backing shall use a backup alarm and/or spotter.

  • Equipment operators are qualified for equipment being operated.

  • Seat belts are being used on equipment and vehicles.

  • Windows and mirrors are clean and have no damage.

  • Lockout/Tag out is used when inadvertent operation may cause injury.

Demolition

  • An Engineering Survey has been completed by a competent person.

  • A JHA and pre-task has been reviewed with all workers involved.

  • Demolition area has been fenced and/or secured.

  • Workers are using appropriate PPE. (EPA selection guidelines: Level A, B, C, or D)

  • Atmosphere is being monitored when required.

  • Workers dealing with abatement have been trained. (Asbestos)

  • Exposure to Asbestos, Lead, PCB, Silica and H25 are controlled.

  • All floor openings are barricaded or covered and marked "HOLE"

  • Explain observations and corrective actions observed during this audit.
