CONFINED SPACE AUDITS

    SCOPE: To ensure all safety precautions have been met whilst working in confined spaces in accordance with SOP 431.

  • Is the confined space safe isolation complete?

  • Has atmospheric testing been done and is this recorded on the entry permit?

  • Is there a standby person at the entrance to the confined space?

  • Does the standby person understand the rescue procedure for this vessel?

  • Has the rescue procedures been explained & understood by persons entering the space?

  • Has the standby person & all people working in the space personal Tag or Lock on the point(s)?

  • Are all people wearing the correct PPE as stated on the work Permit &risk assessment?

  • Has a confined space risk assessment been completed?

  • If operating under a hot work permit, is a fire extinguisher located outside the space?

  • If welding in the space, is the standby person aware of how to operate the power cut off switch to the handpiece?

  • Are all power sources located outside the confined space?

  • Portable electrical equipment connected to a 32 volt transformer located outside the vessel?

  • Are all access ways clear?

  • Is there adequate ventilation?

  • All confined spaces and permit-required confined spaces identified.

  • Confined Spaces properly posted "Danger Confined Space - Do Not Enter."

  • Written confined space entry permit available.

  • Confined space "Entrants" and "Attendants" have been properly trained.
