Introduction and Instructions Introduction: PGH Bricks and Pavers believes the foundation of all successful commercial relationships under HVNL Chain of Responsibility is the identification of critical control points allowing seamless communication, based on a combined commitment to continual internal and relationship improvement by suppliers and PGH Bricks alike. Instructions: 1. PGH Logistics Managers are to complete the audit with the third party provider. 2. The audit tool is to be completed in-line with the Reasonable Enquiry Standard and the PGH CoR Code of Conduct for Transport Contractors. All sections of the audit tool are to be completed, with ticks in the appropriate columns. 3. Any items marked “No” should have a comment included and an action completed at the end of the inspection in the corrective action area. 4. Results of audits are to be uploaded into Sharepoint. Any corrective actions need to be created in Beacon Contractor Management System.

Fatigue

  • Has the transport provider conducted a risk assessment on Fatigue?

  • Does the transport provider have a system to ensure drivers comply with work / rest regulations?

  • There have been no breaches to the regulations (Internal & Regulator) since the last review period.

  • Has the performance management of each breach been documented and addressed?

Load Restraint

  • Does the transport provider have processes to ensure all loads are restraint appropriately?

  • There have not been any “directions to restrain” or infringements for load restraint since the last review period.

Maintenance

  • Does the transport provider have a system to ensure all vehicles are maintained appropriately?
