SET UP/PAD ACCESS

  • CLAUSE 1.1: Drill rig location signs easy to follow from main access (consider ESO). Signage includes Company name, Rig Identification, Arrow showing direction. Adequate signage for multiple track changes. Reflective signs for nightshift.

  • CLAUSE 1.2: Are vehicles parked in designated areas, on the pad or access track. Are vehicle wheel chocks fit for purpose and positioned correctly. Are vehicles parked/positioned in such a way that there are no line of fire concerns or issues, not parked on a slope with work activities being conducted behind or downhill of vehicle.

  • CLAUSE 1.3: Rig induction (visitor) includes hazards as sign posted drill site entry signage.<br>Information in the event of an emergency, muster point.

  • CLAUSE 1.4: 1. Personal protective equipment shall comply with the Iron Ore - Personal Protective Equipment Management Work Practice and Iron Ore - Approved PPE List 2. All personnel shall comply with the Iron Ore - Work Attire and Adornment Work Practice.

  • CLAUSE 1.5: Barricading, demarcation and signage shall comply with the Iron Ore - Barricading, Demarcation and Signage Work Practice with drill rig specific exclusions to be compliant with the Resource Development Exclusion Zones Procedure.

  • CLAUSE 1.6: Drill Site Condition (site and all access roads used are in a clean, safe and orderly condition, drains and sumps where required to the satisfaction of the relevant company representative, holes plugged/capped at completion of drilling, completed checklist for hole completion to be kept at rig)

  • CLAUSE 2.24: All vehicles and equipment entering the Site must have been inspected and approved by the Service Provider’s qualified mechanical inspector. <br>All inspection action items resulting from the inspection must be addressed by a competent and qualified mechanic and/or fitter prior to the equipment being allowed access to the Sites.

DOCUMENTATION AND REGISTERS

  • CLAUSE 2.1: The Service Provider’s operational risk register, must be updated by the Service Provider at least annually <br>The Service Provider must ensure that a plant and operation risk assessment has been completed for each Drill Rig operation prior to commencement of any Services in each year.

  • CLAUSE 2.4: The Service Provider’s contact lists and Relevant Company’s Site emergency response plan and contact list which are subject to audit by the Relevant Company, are at Site and maintained up to date at all times.

  • CLAUSE 2.6: A tropical cyclone preparation procedure which is subject to audit by the Relevant Company, is at Site and maintained up to date at all times

  • CLAUSE 2.7: The Drill Rig manufacturer’s operator manual (required to be stored on the Drill Rig).

  • CLAUSE 2.9: The Service Provider must ensure that one up to date set of the completed safety data sheet is stored at the Drill Rig, one copy is stored at the Laydown area of the Site and one copy is provided to the Site Emergency Management Officer or the Relevant Company’s project office.

  • CLAUSE 2.10: Mechanical equipment register that is updated monthly and includes all Service Provider vehicles and equipment on Site.

  • CLAUSE 2.11: Formal documentation of daily pre-starts, monthly/fortnightly checks for all plant and equipment are at Site and maintained up to date at all times.

  • CLAUSE 2.13: The Service Provider must ensure that an electrical equipment register which is subject to audit by the Relevant Company, is at Site and maintained up to date at all times.

  • CLAUSE 2.14: The Service Provider must ensure that a pressure vessel manufacturer data report which is subject to audit by the Relevant Company, is at Site and maintained up to date at all times. The Service Provider must ensure that pressure vessel design information and drawings which is subject to audit by the Relevant Company, is at Site and maintained up to date at all times.

  • CLAUSE 2.17: The Service Provider must ensure that isolation registers for plant and equipment/procedures and competency certificates which are subject to audit by the Relevant Company, are at Site and maintained up to date at all times.

  • CLAUSE 2.18: The Service Provider must ensure that a personal master and visitors lock register; which are subject to audit by the Relevant Company, are at Site and maintained up to date at all times.

  • CLAUSE 2.19: The Service Provider must maintain at the drill site a lifting equipment register and documentation including all attachment points, subs, slings, winch ropes and pull down/pull back chains and ropes.

  • CLAUSE 2.20: The Service Provider must ensure that a working at heights equipment register including anchor points which are subject to audit by the Relevant Company, are at Site and maintained up to date at all times.

  • CLAUSE 2.21: The Service Provider must ensure that a plant and vehicle risk assessment for each item on Site which is subject to audit by the Relevant Company, is at Site and maintained up to date at all times.

  • CLAUSE 6.1: Permit to Work Requirements must comply with the Iron Ore (WA) Permit to Work Guidance Note A copy of the completed permit to work must be kept at the work area until the work is complete.

  • CLAUSE 6.2: Where there is a risk of falling, appropriate control measures must be implemented.

  • CLAUSE 6.9: Before ANY Working at Height commences, a Rescue Plan must be developed by Working at Height competent people at the work site as part of the Working at Height Permitting process. Rescue plan will take into account; <br>The immediate response after a fall has occurred (by the Standby Person) <br>The equipment required to initiate the emergency response <br>Emergency Contact numbers

  • CLAUSE 6.10: A SWP or JHA for the task must be provided prior to commencing any hot work activity. The Hot Work Permit Issuer and the Responsible Person must not be the same person.<br>Fire Watch must complete RTIO (WA) Extinguisher Use (WBT / classroom) or RTO equivalent.

  • CLAUSE 8.3: All drilling plant and equipment must have a written equipment isolation procedure with steps to completely isolate the specific piece of equipment.

VEHICLES AND DRIVING

  • CLAUSE 4.2: Journey Management shall comply with Iron Ore and Resource Development Journey Management Guidelines.

  • CLAUSE 4.3: Support vehicles shall be all-wheel drive vehicles or track-mounted. <br>Vehicle selection must be fit for purpose. If any part of the journey requires travel on unsealed roads then a 4WD must be used and engaged.

  • CLAUSE 4.7: Loose items shall not be transported in the cab of any vehicle and items that could fall from the vehicle tray must secured.

  • CLAUSE 4.8: Securing loads for transport shall comply with National Transport Commission Load Restraint Guide 2018 Load Restraint Guide for light vehicles 2018.

  • CLAUSE 4.9: All drill rigs and all pipe transporting support vehicles (including sub-contractor vehicles) shall have engineered side uprights, plus front and rear gates fitted to rod bins or cradles. This applies to all applicable equipment, during mobilisation and demobilisation, as well as travelling within and between Sites.

  • CLAUSE 4.10: All light vehicles including the rig crew carrier, drilling supervisor and fitters' vehicles and water truck shall have operational and approved in-vehicle monitoring systems (IVMS) and data from this equipment made available for download and inspection by the Relevant Company’s representatives.

  • CLAUSE 4.11: Tyres are of uniform size, width, load rating, speed rating across each axle and tyres of the same carcass construction fitted to the same axle. <br>Except at tread wear indicators, a tyre fitted to the vehicle must have a tread pattern at least 3.0 mm deep. All light vehicles, trucks, and trailers must have wheel nut indicators fitted to all wheels.

  • CLAUSE 4.12: No unattended vehicle or mobile equipment is to be left idling at any time, unless it is required for the specific task or operation of the equipment. (Must follow Resource Development Un-Isolated work Process). All vehicles on Resource Development sites must be parked in a fundamentally stable position with the vehicle wheel chocked.

  • CLAUSE 4.13: To reduce the risk of injury or, damage to equipment or the environment a spotter must be used. The Service Provider include a Spotter SWP to support the requirement.

  • CLAUSE 4.14: The Service Provider must provide ultra-high frequency radios in all its vehicles and train all Service Provider Personnel in their use and also supply either a high frequency radio or satellite phone. The Service Provider must ensure that the ultra-high frequency and high frequency radios are tuned to the frequencies used by the Relevant Company.

  • CLAUSE 4.16: First Aid Kits (list of contents, all items stocked and in date, easily accessible, located in each vehicle, position close to signage).

PLANT AND EQUIPMENT

  • CLAUSE 5.1: All vehicles must carry composite blocks for jacking for the purpose of jacking drill rigs, sloops, support equipment e.g. Vehicle loading crane and tyre changing.

  • CLAUSE 5.3: All drill rigs shall have a multi-point emergency shutdown system with a minimum of three shutdown points (one each located at the control panel, the left hand side and the right hand side of the drill rig) with clear identification at each shutdown point. Each shut down point must be easily accessible when a drill rig is set up in drill mode. All auxiliary air units and all large volume mud pumps shall have at least two emergency stops, one of which must be accessible from ground level. All other auxiliary drill equipment must have at least one emergency stop.

  • CLAUSE 5.4: Ladders, stairs, fixed / non-motorised platforms, walkways and handrails shall comply with Iron Ore - Ladders, Stairs, Walkways, Fixed and Non-Motorised Work Platforms Guidance Note. All drill rigs and drilling support equipment which are exposed to working at heights (including where there is a potential to fall from one level to another) shall have engineered fixed platforms, walkways, hand rails, self-closing one way gates and ladders designed and constructed in accordance with AS 1657.

  • CLAUSE 5.5: All diesel motors must have a key start system with pull starts disabled. All stationary equipment must include a lockable battery isolator.

  • CLAUSE 5.7: The Service Provider must ensure that each individual Drill Rig operation has a minimum of one complete hydrocarbon spill kit located at the Drill Rig, one complete hydrocarbon spill kit located with the support truck (bulk fuel) and one complete hydrocarbon spill kit located at the Laydown area nominated by the Relevant Company.

  • CLAUSE 5.8: The Service Provider must ensure clear identification and labelling of all: <br>(a) controls; <br>(b) gauges; <br>(c) rated anchor points; <br>(d) emergency stop points; <br>(e) fire suppression points; <br>(f) first aid points; <br>(g) mandatory PPE; <br>(h) hot areas; <br>(i) moving/rotating parts; <br>(j) danger / exclusion zones; <br>(k) rig capacity; <br>(l) fuel tanks; <br>(m) hydraulic tanks; <br>(n) receiver tanks; and <br>(o) water tanks.

  • CLAUSE 5.10: The Service Provider must ensure that guarding is installed to all accessible moving/rotating parts of the Drill Rig.

  • CLAUSE 5.11: Drill Rigs require the ability to limit the main winch, wireline and/or sand line speed and load settings so as to prevent any overrun and not to exceed Safe Working Loads, Working Load Limit and rated capacity. Limiters must be fitted , fully functional and prevent the chance of overrun when pulling rods, lifting suspended loads and /or the overshot.

  • CLAUSE 5.12: Drill Rigs require the ability to limit the main winch, wireline and/or sand line speed and load settings so as to prevent any overrun and not to exceed Safe Working Loads, Working Load Limit and rated capacity. Limiters must be fitted , fully functional and prevent the chance of overrun when pulling rods, lifting suspended loads and /or the overshot.

  • CLAUSE 5.13: Engineered certified overshot safety chains, or slings, and an overshot safety lock system with securing pins must be used for all diamond core inner tube movements to and from the tube rack to the vertical position in the mast.

  • CLAUSE 5.12.3: Wire rope end preparation, fittings, end attachments and splicing must comply with AS 2759:2004 Steel Wire Rope - Use, operation and maintenance.<br>The Service provider shall ensure wireline splicing shall meet verification of competency requirements.

  • CLAUSE 5.15: Mast pivot points and mast hydraulic ram pins must have a locking device to ensure pins remain in place. All attachment points located above head height, which, as a minimum include ram pins, shackles, clevis pins and winch boom attachment points, must have secondary securing devices to prevent falling objects. All rated shackles fitted to a drill rig and support equipment shall be of a four-part configuration and comply with AS 2741:2002. All bolted components above head height shall have a form of secondary retention. <br>R-clips or lynch clips shall not be used for pin retention.

  • CLAUSE 5.17: Drill Rigs have rated double ended hose whip socks fitted to all high pressure air hoses and to the water swivel supply hose. The high pressure air and the water swivel supply hose must be secured back to rated dual anchor points at the supply delivering end by whip socks fitted with four part shackles (ratings referred to in this clause must be in accordance with applicable Australian and Relevant Company standards).

  • CLAUSE 5.18: Sloops have engineered locking devices for truck tray transportation.

  • CLAUSE 5.19: All Drill Rigs shall have fitted functional automatic systems interlocked with rod rotation and moving parts guarding (or cages) at the base of the mast area that prevent access to all rotating and moving parts. Guarding shall prevent access to rotating rods when the mast is in an angled and / or dumped position. (Below the mast structure) Rod Guards must be fitted with interlock systems to prevent or reduce the RPM of rod rotation when the guard is open

  • CLAUSE 5.20: All drill rigs shall be fitted with functional rod and casing (pipe) handlers. When the pipe handler is designed to operate in conjunction with a rod spinner, the rod spinners must be functional and fully operational, when the rod spinner is not in use it must be isolated. Diamond Drill Rigs shall have an automated rod break out capability fitted to eliminate the need to manually break and / or spin out rods, with rod spinners, diamond rod handler or make and break clamps / jaws preferred.

  • CLAUSE 5.21: The control levers of the following components must be designed to automatically return to a neutral setting: a) Winch / mainline, b) Wireline, c) Rig jacks, and d) Mast raise and lower.

  • CLAUSE 5.22: Refuelling nozzles shall be of the automatic shut off type that prevent overfilling. All auto-lock devices must be removed before any Drill Rig enters onto a Site.

  • CLAUSE 5.23: The Service Provider must supply two lighting towers approved by the Relevant Company per Diamond Drill Rig operation for all Double Drilling Shift sessions; or alternatively, provide alternative lighting which meets minimum illumination standards as approved by the Relevant Company.

  • CLAUSE 5.24: Tooling on the drill rig shall meet the requirements of the Hand & Power Tool Safety Work Practice and comply with Banned and Restricted Tool List.

  • CLAUSE 6.3: The Service Provider must incorporate platforms and handrails onto each Drill Rig for ease of servicing, refuelling, inspections and maintenance. These shall be compliant with the applicable Australian standards and the Relevant Company's HSEQ MS requirements. Access to equipment shall be designed and constructed in accordance with AS 1657. Where there is a risk of falling, appropriate control measures must be implemented.

  • CLAUSE 6.4: Walk ways and work platform surfaces must be self-cleaning and non-slip. Free of trip hazards.

  • CLAUSE 6.5: All drill rigs and drilling support equipment which are exposed to working at heights (including where there is a potential to fall from one level to another) shall have engineered fixed platforms, walkways, hand rails, self-closing one way gates and ladders designed and constructed in accordance with AS 1657.

  • CLAUSE 6.6: All permanent working with heights anchor points must be clearly identified, labelled and rated in accordance with applicable Australian and Relevant Company standards

  • CLAUSE 8.2: All plant and equipment in the Drilling Suite must be fitted with lockable isolation points with clear identification.

  • CLAUSE 9.5: The Service Provider must ensure that each individual Drill Rig operation has a minimum of one complete hydrocarbon spill kit located at the Drill Rig, one complete hydrocarbon spill kit located with the support truck (bulk fuel) and one complete hydrocarbon spill kit located at the Laydown area nominated by the Relevant Company.

  • CLAUSE 24.1: The Service Provider must supply and utilise portable bunding for the storage of hydrocarbons at the designated Laydown area or utilise appropriate storage facilities where they exist or are provided by the Relevant Company. The Service Provider must rationalise the number of areas used for hydrocarbon storage.<br>During any refuelling and emergency servicing that may be required at Site, portable bunding, drip trays and absorbent matting must be used.

QUALITY PERFORMANCE

  • CLAUSE 7.1: Sample collection shall be carried out in accordance with the Resource Development Diamond Core Collection and Transportation Guidance Note.

  • CLAUSE 7.2: The Service Provider must ensure that each of the hole walls are washed with a jetting tool as the rods are pulled out of the hole for the purpose of imaging survey, under instructions from the Relevant Company.

  • CLAUSE 7.3: The Service Provider must obtain samples using a standard sample interval of 2.0 metres (m), starting at ground level and marked sample bags reflect the interval drilled.

  • CLAUSE 7.4: The Service Provider must take every reasonable precaution to prevent loss or contamination of the sample.

  • CLAUSE 7.5: The Service Provider must take every reasonable precaution to prevent loss or contamination of the sample. Reference run book/drilling Logs and confirm correct hole depths.

ISOLATION

  • CLAUSE 8.1: A copy of the Iron Ore (WA) Isolation Work Practice must be kept at the drill site.

  • CLAUSE 8.2: All plant and equipment in the Drilling Suite must be fitted with lockable isolation points with clear identification.

  • CLAUSE 8.3: All drilling plant and equipment must have a written equipment isolation procedure with steps to completely isolate the specific piece of equipment.

  • CLAUSE 8.4: For maintenance and operational tasks which involve work where hazardous energies cannot be isolated, the un-isolated work approval process shall be followed prior to commencing work as referenced in the Iron Ore (WA) Isolation Work Practice and Resource Development Working with Live Equipment Process.

  • CLAUSE 8.5: Where it is necessary to work on live/un-isolated equipment for example commissioning, decommissioning, testing, calibration, sampling, adjustments, fault finding and troubleshooting, such work must be carried out in accordance with an approved procedure and controlled by a designated person who is deemed competent and authorised.

  • CLAUSE 8.7: RTIO supplied tags Correct use as required in the Iron Ore (WA) Isolation Work Practice.

  • CLAUSE 8.8: Equipment connected physically connected to equipment (e.g. drill rig to water/aux truck, water truck to water pumps) must raise a personal isolation then as a minimum apply master lock & tag and an out of service tag placing the equipment out of service for operational reasons.

  • CLAUSE 8.9: Operator understanding of the Isolation steps and hazardous energies included in the Area specific isolaton procedures. Clear and Trial step.
