Safety Planning Provisions and Activities Checklist

SITE OFFICE

  • Is Construction Phase Plan in the H&S File on site?

  • Are insurance certificates displayed and in date?

  • Is the F10 Notice displayed and is the information accurate and in date?

  • Is the Health and Safety Law Poster displayed?

  • Are all relevant RAMS available for operatives and Has the RAMS sign off been sheet been signed by all operatives working on site?

  • Is there an Incident Reporting book available in the site office?

  • Are operatives qualifications available in the H&S File?

  • Is site foremans SSSTS/SMSTS Qualification displayed in the site office?

SITE OBSERVATIONS

  • Are there appropriate safety signs in place? e.g. Traffic Routes, Authorised Personnel, PPE etc?

  • Is the site secure, restricting access from the public?

  • Is there a clear Traffic Management Plan displayed?

  • Are these traffic routes kept clear of any obstructions and well lit?

  • Do vehicles have visual and reversing aids fitted where needed?

  • Is the site tidy and well laid out?

  • Are walkways adequate? e.g. Stoned up

  • Are barriers for walkways in-tact? (no gaps)

  • Are Welfare Facilities e.g Toilets, Canteen adequate and are they kept clean?

  • Are First-Aid facilities in place and do operatives know where they are?

  • Is there appropriate lifting equipment available for handling heavy loads? Has it been certified and inspected?

  • Have existing services been identified? (overhead, buried lines)

  • Is there appropriate edge protection in place when working at height or in excavations?

  • Is edge protection adequate and set up correctly? (Minimum 950mm High, Intermediate rail less than 470mm gap & Toe Board)

  • Are operatives on site wearing correct PPE?

  • Are suitable protective measures being used to prevent or reduce exposure to dusts e.g. wood, cement, silica?

  • Is work equipment, plant and machinery maintained in a safe condition and is it inspected regularly by competent persons?

  • Are bucket changing areas adequately fenced off and is signage displayed?

  • Are refuel areas adequately fenced off?

  • Are fuel tanks appropriately bunded and are spill kits available?

  • Are manholes cut down and are they appropriately covered?

  • Is exposed rebar appropriately capped?

  • Are there adequate stop blocks at the mixing bays?

  • Are operatives using correct PPE whilst mixing?

  • Are gulley bags needed? (near water)
