MANDATORY - REQUIRED FOR ALL ITEMS OF PLANT ON A ROADTEK SITE

  • Seat belt(s) are fitted, lights (head, reverse, stop, tail, indicators, rotating)

  • Reversing beeper and horn operational

  • Maximum operating height and reach of plant (for exclusion zones)

  • Vertical reach

  • Horizontal reach

  • Any lifting chains/slings/points or equipment that are to be used: tested, tagged, certified and in date

  • Machinery guard(s) are fitted and secured, where applicable

  • Anchorage points for fall arrest rated (If applicable)

  • Hose burst protection is fitted if plant item is to be used in crane mode to lift loads over 1tonne

  • Visual check of tyres/tracks, wheel nut indicators (if fitted): In comments, describe the condition. Are the tyres/tracks free of obvious defects?

  • UHF radio or other type communication (If applicable)

  • Noise rating of plant available to operator

  • Check for valid registration, logbooks, operator's manual & defect book, maintenance and service regime.

  • Daily operator plant specific checklist is provided and there is a requirement and area for the operator to sign off these are being done daily (Must have all items)

  • Biosecurity hygiene form provided

  • Plant risk assessment provided

Plant/ Equipment Specific

  • EPWs

  • Evidence of all types of EWP inspections:<br>- Routine inspections: at least every 3 months<br>- Periodic inspections: at least every 12 months<br>- Major inspections: at least after 10 years of service, and every 5 years thereafter

  • Earthmoving Equipment

  • If plant is earthmoving equipment, where required, is fitted with roll over protection (ROPS) and/or falling<br>objects protection (FOPS) and fitted with a compliance plate stating it’s compliance to AS 2294 (alternative<br>compliance may be ROPS- ISO 3471, SAE J1040 or FOPS -ISO 3449, SAE J231 or J 1043 or TOPS AS 4987 or<br>ISO 12117)

  • The person with management or control of earthmoving
    machinery at a workplace must ensure that the machinery is
    NOT used unless it is securely fitted with a protective structure
    (ROPS & FOPS) and fitted with a compliance plate.

  • Concrete Pumping Equipment

  • Concrete pumping equipment is fitted with the required compliance plates.<br>Placing boom and outriggers: manufacturer fixed plate, annual and six yearly inspection date fixed plate. Concrete<br>pump: manufacturer specification fixed plate.<br>If applicable: over length and overweight permits, fixed motor vehicle modification plate showing any applicable<br>modification codes.

  • PREFERABLE - FOR ALL ITEMS OF PLANT ON A ROADTEK SITE.

  • Consider the risk of these items not being on the plant (or checked) and document.

  • Check for obvious leaks of hydraulic fluid or water

  • Mounting / access step(s), ladders, work platforms fitted, appear in good working order

  • Trailer coupling device secure and fitted with safety chains (If applicable)

  • Mirrors

  • Fire extinguishers – Are they fitted and in Date?

  • Walk around plant and list existing defects / hire conditions / general conditions: (body panels, paint, cutting edges and teeth, general damage)
