Review Questions

  • Are plant/vehicle routes delineated?

  • Are HV and LV routes separated and well identified?

  • Is radio communication required between plant/vehicles?

  • Are radio channel(s) well sign posted?

  • Monitor the radio channel(s) for a period - are comm's requirements being followed?

Spotters

  • Are spotters being used on-site? if NO go to next section.

  • Spotters are isolated from plant/vehicle movements?

  • Have good line-of-sight of plant/vehicles?

  • Are identifiable from general site workers?

  • Have radio communications?

  • Are vehicles/plant complying with their instructions?

  • Only perform spotter duties?

Review Questions continued

  • Have LV parking areas been established at the work area(s) which comply with Separate Yourself guidelines?

  • Have parking area(s) been established on risk management principles?

  • If hazards have not been eliminated are they controlled?

  • Have HV parking area(s) been established at the work site(s)?

  • Are HV parking areas level?

  • If HV parking area(s) are not level, has the risk of run-away been controlled?

  • If HV/plant are temporarily parked on-site, have they been turned off, gear box in "P" or an appropriate gear and handbrake applied?

Safety Devices

  • All plant/vehicles fitted with a roof mounted (or similar) rotating/flashing, amber light?

  • Audible, reversing squawker or buzzer?

  • Reversing camera? - NOT a requirement under TPR-G22

  • Reversing light? - NOT a requirement under TPR-G22

Safety Devices - Nonconformities

  • If either plant or HV have an inoperable or inaudible reversing alarm, they must not continue to operate and should be removed from site. As per TGR-G22

Administrative Controls

  • Have pre-start checks been completed on plant?

  • Are plant movements and associated hazards discussed at pre-start meetings?

  • Plant operators have undertaken a VOC for this project?
