    There have been a number of HME Access System incidents that have been assessed as having significant potential.Following the incident investigations we need to audit other HME to stop them recurring. The attached questions have been identified as part of the investigations and from syndications across the business. Each asset and Fleet type may be different, due to when they have been purchased and or subsequently modified, so it's important to ensure all questions are answered asset by asset.

  • Does the park brake release if the access system is not fully stowed?<br><br>If yes detail action required in comments

  • Is safe access / Egress from the cab reliant on correct shutdown procedure ? i.e. platform descends when park brake is set?<br><br>If yes note actions required in comments
