  • Rooftop Penetrations<br>Capable of supporting at least twice the maximum load expected to cross over the cover.<br>Shall be secured when installed to prevent displacement.<br>Shall be marked with word “HOLE” or “COVER”.<br>

  • Fall Protection - <br>Tied off at 6' or greater. Harness, Lanyard, Self-Retracting Lifeline, Horizontal Lifeline inspections. Elevator Shafts, Floor/Wall Openings, Guardrails, Floor Perimeter, Roof Perimeter.<br>Standard guarding systems include –<br>i) A top rail between 39” and 45”.<br>ii) A mid rail halfway between the top rail and the work surface.<br>iii) Toe boards when falling material may cause a hazard.<br>iv) Window openings must be guarded if they are opened at 39” or below.<br>v) Hole openings 2” or greater must be covered, secured and clearly marked “HOLE”.<br>Access /Egress avenues shall be kept clear of all debris.<br>Inspect all fall protection daily and before use. If it’s defective remove it from service immediately. DO NOT USE DEFECTIVE FALL PROTECTION EQUIPMENT!<br>

Fire Protection

  • Only NFPA approved containers used for fuel?

  • Flammable materials removed from job site properly?

  • Adequate number of fire extinguishers in place?

  • Fire extinguishers monthly inspections documented and have decals in place?

  • Fire extinguisher inspected by user prior to use?

Welding

  • Fire extinguisher present and inspected

  • Hot work permit visible and correct

  • Proper PPE being utilized by welder and helper, Welding mask, leather gloves, proper attire, long sleeve shirt with collar, long pants not cuffed, etc.

  • Weld screens being used to prevent flash to others.

Ladders

  • Do extension or job built ladders extend 36 inches above laning?

  • Are spreader bars locked on step ladders?

  • Are employees utilizing ladder correctly? Correct ladder for the task?
