Instructions/Comments

  • 1. Completed audits are to be emailed to Diane Littlewood diane@safetasks.com.au and Jeff Alcott jeff@safetasks.com.au 2. Audits are to be carried out a minimum of one per shift during shut downs. 3. Type obervations and necessary actions in the text box provided at the end of each section. 4. Use the action plan text boxes at the end of the audit tool to record significant items requiring promt action by the relevant supervisor or manager. 5. Ensure all questions are answered with Yes, No or N/A 6. Media boxes have been provided throughout the audit tool to provide photo evidence of specific items. 7. Do not edit or change the audit tool without the permission of the Managing Director or the Safety & Training Manager.

Confined Space General

  • Is all safety equipment required in good condition and within inspection dates

  • Is all scaffold within the confined space tagged with a relevant scaf tag and is within date

  • Is there an adequate means for entry and exit

  • Gas cylinders are positioned outside the confined space

  • Has a risk assessment been completed for the confined space

  • Is there an adequate means of communication with the persons inside the confined space

  • Where applicable have all persons signed onto the Work Permit, Work Party or other relevant document

Standby Person General

  • Is the standby person outside and in close proximity to the confined space

  • Does the standby person know how to raise an alarm in an emergency

  • Does the standby person have a means to raise an alarm with the persons inside the space with those that may not have direct communication with the standby person outside e.g. horns, whistles, bells etc

Entry Permits

  • Have all person exiting the confined space signed off the entry permit

  • Has the standby person signed onto the entry permit proir to any person entering

  • Has the entry permit been revalidated for the shift

Hot Work

  • Is hot work being carried out in the confined space and are there fire extinguishers and/or hose reels close to the work area

Gas Detection

  • Has the gas detector been calibrated within the last 6 months

  • Has gas detection been completed prior to persons entering the confined space

  • Have the gas test recorded any readings above the exposure standard TWA during the shift

  • Has the gas detector been bump tested within the last 24 hours

  • Has the gas detection been carried out in intervals as per the risk assessment requirements

Working At Heights

  • Is the standby person required to work at heights

  • Is the standby person required to wear a harness when working at height
