Osha And General Requirements

  • Are First Aid kits on site & stocked?

  • Do employees know where the first aid kits are located?

  • Is water adequately supplied?

  • Are all employees trained in incident reporting procedures?

  • Is incident reporting conducted according to procedure?

  • Are employees reporting near miss incidents?

  • Is PPE available & properly used?

  • Are CAZ's being used properly? (Tags, access gates closed, put up properly)

  • Is there adequate lighting in work area?

  • Do all containers have proper SDS labeling?

  • Are chemicals used being reviewed with employees prior to use?

Job Safety Analysis

  • Does the JSA cover all the associated hazards for the tasks?

  • Does the JSA specifically discuss all possible eliminations or controls?

  • Have all employees signed, acknowledge, and understand the risks and hazards associated with the task at hand?

  • Has the JSA been Documented?

Fall Protection

  • Is fall protection provided/used for fall prevention in excess of 6ft?

  • Are the fall protection/arrest systems used properly?

  • Are the employees trained in the proper use of fall protection equipment/arrest systems?

  • Are the fall protection/arrest systems inspected/stored and in good working condition?

Tools

  • Do all tools have the proper guards on them?

  • Are all electrical cords & GFCI's in good working condition?

  • Are all tools being inspected and stored properly?

  • Are all tools used overhead properly thethered?

  • Is HEPA vacuum w/shroud being utilized w/all hammer drill operations?

  • Is a wet saw being used with plastibaton/saw cutting operations?

  • Is the right tool being used for the job being performed?

Electrical

  • Are temporary power cords protected from all foot/vehicular traffic?

  • Is LOTO equipment onsite used and documented?

  • Is there a written LOTO procedure?

  • Are the authorized personnel identified, trained, and qualified to perform a LOTO procedure?

  • Are LOTO tags properly filled out & up to date?

  • Are there covers or faceplate on the boxes?

  • Are all damaged items tagged & taken out of service?

  • Are all extension cords elevated & off the ground?

  • Are all extension cords in good working condition?

  • Is all temporary power protected with a GFCI?

Equipment

  • Are all operators trained & certified?

  • Are daily equipment inspections being performed prior to operating?

  • Are operators using seatbelts?

  • Does equipment have a functional back up alarm?

  • Is all equipment being operated at safe and appropriate speeds?

  • Is the equipment being utilized for what it is made for?

Confined Space

  • Has the confined space atmosphere been tested for LEL/O2C0/H2S or other toxins?

  • Is all required PPE in place and used properly?

  • Is there a method of communication between the attendant?

  • Are the Site-specific requirements/procedures in place?

  • Is a competent person in place? (Supervisor)

  • Have all required permits been completed?

  • Have all entrants & attendants been properly trained?

  • Has a rescue plan been established?

  • Is a rescue person available at the confined space location?

  • Is the emergency retrieval system adequate for the employees evacuation?

  • Has it been inspected?

  • Is there an attendant monitoring the entrants at all times?

  • Is there a ventilation system set in place?

Ladders

  • Are all ladders being used and inspected prior to use?

  • Are spreaders on ladders dawn and locked in place?

  • Are the ladders being used appropriate for the task?

  • Do the extension ladders extend 3 feet above the leading edge?

  • Are extension ladders secured at the top and bottom?

  • Are doorways barricaded as necessary when using a ladder around them?

  • Are employees trained to use a ladder correctly?

  • Is a proper fall protection used when required?

  • Are ladders used as a last means necessary?

  • Are ladder permits being utilized for the standard A frame and standard extension ladders?

  • Are 3 points of contact being used when going up and down the ladder?

Aeriel Lifts

  • Are employees trained/certified on equipment?

  • Have certifications been provided for the operators?

  • Are all lift/platform safety devices operational?

  • Is all fall protection used properly?

  • Are lifts/platforms operated according to manufacturer?

  • Are daily equipment inspections completed according to procedures?

  • Are the lifts clear of trash and debris?

  • Are both feet planted while in Aeriel lifts?

Spotters

  • Are spotters properly trained to perform their duties?

  • Are they engaged with what's going on?

  • Are they providing proper hand signals for their operator?

  • Is a good form of communication established between the spotter and the operator?

  • Do they stop operations when vehicles, pedestrians, or SIMOPS come close to the work they're performing?
