Inspection Findings

Scaffolds inspected and their location

  • List all scaffolds that have been inspected with short description and their location on site.

Types of scaffolding on site

Independent

  • Is there an independent scaffold on site?

  • Is the scaffold built to a design drawing or TG20 current edition?

  • Are the standards correctly spaced?

  • Are the standards vertically positioned?

  • Are the transoms correctly spaced?

  • Are the ledgers and transoms level?

  • Are the ledgers, and standards correctly staggered or spliced?

  • Is the ledger bracing correctly installed?

  • Is the façade bracing correctly installed?

  • If check fittings are required, are they installed?

  • Does the boarding extend beyond the butting tubes no less than 50mm and no more than 150mm?

  • If there are short boards under 2.4m have they been secured against tipping or displacement?

  • Is the working platform free from gaps exceeding 50mm?

  • Are the tops of the standards the correct height to avoid trip hazards?

  • If there is a service gap, is it as small as reasonably practicable?

  • Are the guardrails correctly installed?

  • Are all the guardrails installed at the correct height (950mm & 470mm)?

  • Are the toe boards correctly installed?

  • If required, is there an internal handrail or other safe system of work in place?

Beam sections

  • Are there any beam sections within the scaffold?

  • Are any supporting towers erected correctly?

  • Are the beams connected using load bearing couplers?

  • Are the lacing tubes correctly spaced?

  • Are the lacing tubes fixed with load bearing couplers?

  • Are the puncheon tubes connected to the top and bottom chords by load bearing couplers?

  • Are check fittings correctly installed?

Loading bays

  • Are there any loading bays on site?

  • Are the loading bays built to design drawing or TG20?

  • Are the standards correctly spaced?

  • Are the transoms correctly spaced?

  • Is the bracing correctly installed?

  • Are handrails and toeboards correctly fitted to the working platform?

  • If there is a loading bay gate, is it correctly fitted?

  • Is the platform free of gaps greater than 50mm?

  • Are the check couplers installed correctly?

  • Is the SG4 handrail in place on non working lifts?

Pavement gantry

  • Is there a pedestrian gantry built into the scaffold?

  • Is the gantry built to design drawing or TG20?

  • Is there sufficient head room and width for pedestrians to pass safely under the scaffold?

  • Is there sufficient signage to warn pedestrians if operatives are working above?

  • If protective foam is fitted, is it fastened correctly and in good condition?

  • Is the scaffold on highlighted tredders (plastic sole pads)?

  • If double boarding with sheeting between is required, is it fitted correctly?

  • If required, is plan bracing fitted correctly?

Birdcage

  • Are there any birdcage scaffolds on site?

  • Are the sole boards and base plates correct?

  • Are the standards correctly spaced?

  • Are the standards vertical?

  • Are the tops of standards the correct height to avoid trip hazards?

  • Are the structural transoms in place within 300mm from the node point?

  • Are the transoms correctly spaced?

  • Are the ledgers and transoms level?

  • Is the ledger bracing and façade bracing correctly installed?

  • If check fittings are required, have they been installed?

  • If required, are handrails and toeboards fitted correctly to the working platform?

  • Is SG4 handrail in place on non working platforms?

  • Is there safe access to the working platform?

  • Is the platform boarded correctly, with no gaps larger than 50mm?

Tower scaffold

  • Are there any tower scaffolds on site?

  • Is the scaffold built to a design drawing or TG20

  • Are the standards spaced correctly?

  • Are the standards vertical?

  • Are the transoms correctly spaced?

  • Are the ledgers and transoms level?

  • Is the ledger bracing correctly installed?

  • Is the façade bracing correctly installed?

  • Are the joints in standards staggered correctly or spliced?

  • If check fittings are required, are they installed correctly?

  • Does the boarding extend beyond the butting tubes no less than 50mm and no more than 150mm?

  • If there are short boards under 2.4m have they been secured against tipping or displacement?

  • Is the working platform free of gaps which exceed 50mm?

  • Is there a service gap and is it as small as reasonably practicable?

  • Are guardrails correctly installed?

  • Are all the guardrails installed at the correct height (950mm & 470mm)?

Edge protection

  • Are there any edge protection handrails on site?

  • What type of handrails are installed?

  • Are the handrails fitted correctly?

  • Does the handrail comply with the current regulation or a design drawing?

  • Are the standard spacings correct?

  • Are the standards level?

  • If check fittings are required, are they installed correctly?

  • Are the handrails at the correct height, with no gaps larger than 470mm?

  • Are the handrails level?

  • Have the handrails been installed using load bearing couplers?

Scaffold foundation

  • Are the foundations firm, and adequate to take the load of the scaffold?

  • Are the sole boards correctly positioned, and the correct size?

  • Are the baseplates correctly positioned?

Scaffold components.

  • Are the scaffold components free from damage?

  • Have the correct components been used?

Scaffold boards

  • Are scaffold boards used?

  • Are the scaffold boards free from significant defects, such as cuts, notches, burns, dents?

  • Are the boards generally lay flat and straight and free from warping or twisting?

  • If the boards have been cut or notched, has a transom been installed either side of the notch?

Access and egress

Staircase access

  • Are there any staircases on site?

  • Are the staircase built to the manufactures instructions?

  • Are the components free from damage?

  • Is access clear and safe?

  • Are the staircases tied at every lift, through both sets of standards?

Ladder access

  • Is there ladder access provided on site?

  • Are the ladders in good condition, and free from defects?

  • If there is a base lift ladder, is it on firm, level, and stable ground?

  • Are the ladders set at the correct angle?

  • Do the ladders extend to the correct height above each platform?

  • Are safety gates, or internal ladder traps fitted at each platform or access point where required?

  • Are the ladders correctly secured with no obstructions?

  • Are the ladder openings the correct size?

Ties

  • Does the scaffold require ties?

  • Have the ties been installed with load bearing couplers?

  • Are the tie tubes correctly installed?

  • Anchor type

  • Base material

  • Tie Pattern?

Falling objects

  • If brickguards are required, have they been correctly installed?

  • If debris netting is required, has it been correctly installed?

  • If sheeting is required, has it been correctly installed?

  • If a fan is required, has it been correctly installed?

Scaffolding housekeeping

  • Are scaffolding components stored neatly and correctly on site?

General site housekeeping

  • Are the scaffold platforms free from debris or loose materials?

  • Have the tradesmen's materials been stored correctly on the working platform?

  • Are the tradesmen's materials loaded correctly within the safe working load limit?

  • If there are tradesmen's materials stored on the platform, is there a safe route of access around the working platform?

Scaffold tags

  • Is there a scaffold tagging system in place?

  • Have all the scaffold tags been updated and signed off?

  • If a scaffold tag has not been updated and signed off due to an issue with the scaffold, then the scaffold tag must be removed to show a do not use sign. Scaffold users must be told that the scaffold is not fit for purpose and asked to leave the scaffolding. Access should then be prevented to the scaffold before leaving the area. The findings must be immediately reported to the principal contractor / hirer of the scaffold before leaving site.

Conclusion

  • Is the work area (including on or around the scaffolding) free of significant risks to health and safety?

  • List any corrective actions taken in respect of findings identified in the report?

  • Names of the persons that the inspection report has been discussed with.

  • Any findings, where there is a risk to the health and safety of others must be discussed with the principal contractor or hirer of the scaffold before leaving the location. If the principal contractor or hirer is not in attendance then they must be contacted as soon as possible. This is a legal requirement under the Health and Safety at Work act 1974. If no contact can be made on site, contact Omega Scaffolding before leaving site to discuss findings. If possible the scaffold should be made safe, or access to the area prevented until a time that it becomes safe to do so.
