Part 3 – PERMIT ACCEPTANCE (Contractor Supervisor to complete)

1. The Contractor agrees to comply with Legal requirements as maybe necessary to ensure the works covered by this permit are carried out in a manner that is safe and without risks to the environment or health and safety, including under the relevant WHS Legislation

2. The Contractor Supervisor has read and understood Scentre’s Groups requirements for these works as defined in the Health and Safety Management plan and the relevant High Risk Work Guide (if applicable) and agrees to comply with those requirements

3. The contractor named in Part 1 of this permit, must ensure that all persons involved in the hot works have been consulted about and are conversant with this permit and the relevant Safe Work Method Statement (if applicable). The Contractor warrants that all workers involved in the works associated with this permit have received sufficient training and instruction and will be appropriately supervised to enable them to perform their works in a manner that is safe and without risks to the environment or persons health & safety.

The Scentre Group issuer of this permit must be trained in the use of the permit and be authorised to issue the permit as defined in the Projects Health and Safety Management Plan.
PART 5 – FIRE WATCH COMPLETED AFTER 60 MINUTES AFTER WORKS COMPLETED <br>(Contractor Supervisor to complete)

1. The Fire Watch must be carried out by a person who remains in the immediate vicinity of the works for the duration(s) stated on this permit to monitor the area to ensure that there is no outbreak of fire. The person must be trained in fire hazards and fire control and have fire-fighting equipment closes at hand (appropriate for the type of works being undertaken).

2. The contractor named in Part 1 of this permit, must assess the risks associated with the works and implement an extended Fire Watch, where required.
FIRE CHECK COMPLETED AFTER 60 MIN (Contractor Supervisor must confirm, that a daily Fire Watch inspection has been conducted after 60 minutes of the works being completed.
