Paperwork Review

On-Boarding

  • Day 1 HSE new Hire Packet is completed for employees hired within a year prior to inspection - including PPE check-list, HAZCOM, and Fire Extinguisher training.

Facility Compliance Plans

  • Facility security plan is present and updated within the past year.

  • Facility has Fire Safety Analysis present on-site

PHAs

  • Personal Hazard Assessments completed and logged for all employees? (Spot checks to determine basic completion by employees, and review by managers)

  • PHA documentation completed appropriately (necessary info filled out on each PHA)

Facility Inspection Checklists

  • Monthly Proapne Bulk Storage Inspections are complete

  • Monthly Petroleum Bulk Storage Inspections are complete

Vehicle Inspections

  • Monthly "Creep Test" paperwork is completed for all bobtail trucks

  • Driver Vehicle inspection reports are completed for all vehicles at the facility; spot check prior 90-days, as appropriate (based on use)

  • Driver Vehicle Inspection Reports are filed appropriately on-site, with a dedicated folder for each truck.

Sensit Meters

  • Sensit meters provided to all personnel that light pilots as a part of their jobs.

  • Documentation has been completed for monthly calibration/ bump test of Sensit meters used at this facility. - most recent log present onsite

Propane Operations

  • Leak Checks performed appropriately for last 5 out of gas calls

PPE

  • Facility has a stock of PPE on-site, and easily accessible by safety sensitive employees
