General Site conditions and Housekeeping

  • Is the site organized and clean?

  • Is safety signage in place?

  • Is Accident Prevention Documentation current (weekly meetings, weekly site walks, orientations)?

  • Is the first aid kit adequately supplied?

  • Are Material Safety Data Sheets available on site?<br>

  • Is the required site personal protective equipment in use?

  • Are all vertical hazards capped? Includes rebar, curb stakes, conduit etc.

  • Is work lighing adequate?

  • Overhead hazard check.

  • Site access clear and marked?

  • Are fire extinquishers charged and tagged with current dates?

  • Temporary power and extension cord check.

  • Is fuel stored properly and are no smoking signs posted appropriately?

  • Inspect all masonry and tilt up bracing.

  • Are access points to building clear of debris and tripping hazards?

  • Do back up alarms appear to be working on operated equipment?

  • Inspect for protruding nails or other puncture hazards.

  • Are oxygen and combustibles stored separately? (Acetylene)

Ladders

  • Inspect all ladders for damage.

  • Extension ladder check. Secured adequately? Set at the proper angle? Do they extend a minimum of 36" above the landing area?

  • Stepplader check. Are any being used as leaning ladders? Is anyone standing too high on a ladder? Are ladders set on stable surfaces?

Scaffolds

  • Is the scaffolding set on a firm base?

  • Is there adequate and safe access to the scaffold?

  • Is the scaffold guardrail adequate?

  • Are platforms levels completely planked and adequately supported?

  • Is the scaffolding adequately attached to the structure if required?

  • Is the scaffolding a safe distance from electrical hazards?

  • Are workers protected from falling objects?

  • Are platforms and walkways free of debris and tripping hazards?

Vehicles/Mobile Equipment

  • Are seatbelts provided and being used?

  • Are any vehicles traveling at an excessive or unsafe speed?

Fall Protection

  • Inspect fall protection equipment.

  • Floor and wall openings planked over to barricaded?

  • If in use, is fall protection equipment being used correctly?

  • Are leading edges protected?

  • Are warning lines being used properly? 15' back from roof or deck edge.

  • Observe safety monitor system if in use. Warning line 6' back from edge.

  • Inspect all anchor points.

Excavation and Trenching

  • Are trenches sloped and/or shored properly?

  • Is access provided in treches over 4'/ every 25' minimum?

  • Are barricades or caution tape in place per the affected area?

  • Inspect trench walls for distress.

  • Slopes covered or protected when necessary to prevent erosion.

Protection of the Public

  • Are construction areas barricaded from public access?

  • Inspect fences, locks and gates.

  • Pathway lighting adequate?

  • Traffic control devices used when appropriate?

  • Pedestrian walkways free from debris and construction materials?

  • Pathway elevation changes marked?
