Are the drivers properly licensed and authorised to drive the vehicles?

Are the vehicles capacity and configuration adequate to transport personnel?

Safety rails adequate and in sound condition?

Fuels, chemicals and tools safely transported

Where fuels, tools and chemicals are transported on same vehicle as personnel, are there adequate measures in place to guard against ignition, contamination or injury?

Underfoot conditions compatible with system of work?

Refuse collection and disposal facilities available?

Is spillage of chemicals and fuels controlled?

Arrangement or equipment available to clean HCS spillages

Sufficient clean drinking water available for all employees?

Is appropriate PPE supplied and used?

Are tools and equipment in good order?

System in place to maintain tools in field or are replacements available in field?

Are washing facilities available for chemical handlers?

Are safe working distances maintained?

Is the operation laid out and conducted in accordance with BOP for the company?

Are warming fires controlled?

Is smoking controlled?

If fuel is used for the operation, are adequate fire extinguishers available on site?

Has a Risk assessment for the job in hand been conducted?

Is there and evacuation plan in place?

Does the plan take cognizance of the location and access routes?

Is there a qualified first aider on site?

Is the first aid kit compliant with legislation and is it adequately filled?

Are there adequate communications available on site to ensure comms to base of operations?

Are drivers licensed, trained and authorized?

Are all machine guards on power units and ancillaries in place and maintained?

Are regular safety talks held and recorded?

Provision made for safe storage of personal belongings?

Is the a safe system for cleaning used chemical equipment?

Are empty chemical containers returned to supplier(Chain of custody)?

Any non operational unsafe conditions observed?
