Site Management

  • Are suitable and sufficient barriers, fencing and perimeter controls in place for the works?

  • Are there suitable and sufficient first aiders on site and do they hold valid and in date training?

  • Are works vehicles parked safely and legally?

  • Are works vehicles in good condition and fit for purpose?

Health and Safety Documentation

  • Where applicable, is the below documentation available on site, signed (where required) by all individuals, and is it suitable and sufficient for the activities being undertaken?

  • Does all required fall arrest equipment have a valid and in date thorough inspection by a competent person?<br>(EG Latchway, Sala, Soll, Metreel)

  • Does the structure and fall arrest system have valid and in date safe to climb certification?

  • Is rescue equipment (EG rescue kit, first aid kit etc.) suitably located?

  • Does the rescue kit have a valid and in date thorough inspection by a competent person? (including tamper seal)

  • Are tools and equipment being suitably tethered?

  • Is a suitable and sufficient drop / exclusion zone in place?

Working at Height - Rooftop

  • Do all required individuals have valid and in date rooftop safety training?

  • Does the rooftop safety kit have a valid and in date thorough inspection by a competent person?

  • Is there suitable and sufficient edge protection / falling materials protection?

  • Have all fragile surfaces been identified and suitably protected?

Working at Height - Ladders

  • Do all required individuals have valid and in date ladder safety training?

  • Do all ladders being used on site have evidence of thorough inspection that is valid and in date?

  • Are all ladders setup and being used correctly?

  • Are non conductive, insulated ladders in use where working near electrical hazards?

Radio Frequency (RF)

  • Do all required individuals hold valid and in date Radio Frequency (RF) hazard awareness training?

  • Has the site been isolated correctly?

  • Has a Radio Frequency (RF) sweep of the working area been carried out using an RF monitor?

  • Is there suitable and sufficient RF monitors in use on site?

  • Do all RF monitors in use on site have valid and in date certificates of calibration?

Streetworks

  • - Streetworks sites MUST have a qualified supervisor appointed to oversee the works
    - Operatives MUST be on site at all times during work activities
    - The supervisor does not need to be on site at all times but MUST be able to suitably and
    sufficiently carry out those duties
    - One person cannot cover the duties of operative and supervisor at the same time
    - Operatives and Supervisors MUST hold valid and in date Streetworks Cards and have
    them accessible on site at all times
    - Anyone acting as a supervisor or operative MUST hold the relevant streetworks units and
    can only perform the activities to which they are fully qualified (as detailed below):

  • Do all required individuals hold valid and in date streetworks cards with required units?

  • Where an EToN notice is required, is it available on site and being adhered to?

  • Is a statutory information board in place as required by the streetworks code of practice?

  • Is the TM / PM set up in line with the plan provided?

  • Is signing, lighting and guarding suitable and sufficient for the site location and activities being undertaken?

MEWP's

  • Does the MEWP operator have valid and in date training? (EG IPAF)

  • Is there evidence of a 6 monthly thorough examination (by a competent person) for the MEWP?

  • Is there a suitable and sufficient Method Statement and Risk Assessment on site for MEWP activities?

  • Is a suitable rescue plan in place (including designated person familiarised in MEWP controls on the ground) in case of equipment failure or emergency?

  • Is the MEWP correctly positioned and being operated in accordance with RAMS / plans?

Scaffolding

  • Has scaffolding been erected, altered or dismantled by operatives with valid and in date training? (EG CISRS)

  • Is there evidence of scaffolding inspection by a competent person?<br>(EG once erected, modified and not exceeding every 7 days)

  • Has scaffolding been setup correctly?

Excavations and Buried Services

  • Are utility plans available on site?

  • Have all services been identified and suitably marked on site?

  • Are safe digging practices being used on site?

  • Do Cable Avoidance Tools (CAT) have valid and in date certificates of calibration?

  • Do all individuals using Cable Avoidance Tools have valid and in date training?

  • Is there safe access and egress in place for the excavation?

  • Are all excavations suitably and sufficiently controlled? (EG shored, battered, spoil stored safely etc.)

  • Are insulated tools being used on site? (if digging near electrical services or equipment)

Lifting Operations

  • Does the Appointed Person (AP) or Lift Supervisor (LS) hold valid and in date training? (EG CPCS)

  • Does the crane operator have valid and in date training? (EG CPCS)

  • Do all slingers / signallers have valid and in date training? (EG CPCS)

  • Is there a Lift Plan available on site for lifting operations, including specific Risk Assessment and Method Statement?

  • Does the Lift Plan detail the correct lifting equipment and does the documentation match the equipment on site?

  • Does the crane have evidence of thorough inspection that is valid and in date?

  • Do all lifting accessories (EG slings, chains etc.) have evidence of thorough inspection that is valid and in date?

  • Is the crane and equipment positioned correctly and being used in accordance with the Lift Plan / RAMS?

Electrical

  • Do all electrical engineers have valid and in date training? (EG 18th Edition, ECS card?)

  • Has the site been isolated and locked off correctly?

  • Do all required tools (EG multitesters, earth testers etc.) have a valid and in date certificate of calibration?

  • Are insulated tools in use in accordance with IEC 60900 and EN-0900?

Hazardous Substances

  • Do all individuals using hazardous substances have valid and in date COSHH training?

  • Is there a COSHH pack (EG COSHH Assessment and Material Safety Data Sheet) in place for all hazardous substances being used on site?

  • Are the safety measures as detailed in the COSHH pack being adhered to on site?

Plant Vehicles

  • Do plant vehicle operators have valid and in date training? (EG CPCS)

  • Do plant vehicles have evidence of thorough examination / inspection by a competent person?

Asbestos

  • Do all individuals on site have valid and in date Asbestos Awareness training?

  • Has the Asbestos Register / Management Plan been reviewed and communicated?

  • Has the asbestos hazard potential been managed adequately?

Environment

  • If waste is being produced on site, is it being segregated and stored effectively?

  • If waste has been collected, has a Waste Transfer Note been provided?

Tools and Equipment

  • Are all hand / power tools in use fit for purpose and in suitable condition?

  • Do all required tools / equipment have evidence of servicing and maintenance?

  • Do all required electrical items have evidence of valid and in date PAT testing?<br>(EG chargers, extensions, transformers, power tools etc.)

  • Where in use, do all required individuals have valid and in date abrasive wheel training?<br>(EG grinders)

Manual / Mechanical Lifting

  • Do all required individuals have valid and in date manual handling training?

  • Do all required individuals have valid and in date rigging and lifting training?

  • Does all lifting equipment / accessories in use have evidence of valid and in date thorough inspection by a competent person?<br>(EG ropes, slings, shackles, chains, karabiners etc.)

Working at Height - Rope Access

  • Do all rope access operators / supervisors have valid and in date training? (EG IRATA)

  • Do all rope access operators / supervisors hold valid and in date Working at Height medical certificates?

  • Does all rope access equipment have a valid and in date thorough inspection by a competent person? <br>(EG ropes, karabiners, descenders, slings etc.)

  • Is rescue equipment (EG rescue kit, first aid kit etc.) suitably located?

  • Does the rescue kit have a valid and in date thorough inspection by a competent person? (including tamper seal)

  • Are tools and equipment being suitably tethered?

Mobile Access Towers

  • Have mobile towers been assembled, altered, dismantled, inspected and used by suitably trained persons? (EG PASMA)

  • Has the mobile access tower been setup correctly and in a safe / suitable location?

  • Reason for Abort:

  • Is an Oil Spill Kit available on site, in good condition?

  • Is there a BS-8599 first aid kit available on site with adequate stock?

  • Is there a fire extinguisher available on site, in good condition with evidence of inspection?

  • Is there a sharps kit available on site, in good condition?

  • Is there a HSE box / bag available on site, with adequate stock?<br>Content should include:<br> - 6x standard safety gloves<br> - 3x cut resistant gloves<br> - 20x ear defenders<br> - 3x safety glasses / goggles<br> - 6x FFP3 dust masks<br> - 1x sun protection cream

  • Is there suitable and sufficient welfare arrangements in place for the works?

  • Is site welfare clean and tidy?

  • Is there suitable means of communication in place between all individuals on site?

  • Are attendees aware of the latest QHSE Communication?

  • Do all required individuals have access to and understand how to use Microsoft Teams (Central)?

  • Is there suitable and sufficient PPE on site for all required workers, and in good condition?

  • Does all required PPE have a valid and in date thorough inspection by a competent person?<br>(EG climbing helmets, climbing harnesses, lanyards etc.)

  • Are contractors being used on site listed as "Approved" on the Approved Contractor List?
