  • Smoking, when permitted, is in designated areas?

  • Are fire hydrants free from obstructions? (If applicable)

  • Are exits marked and free from any obstructions and are not locked?

  • Are flammable materials stored properly?

  • Are emergency numbers posted?

  • Flammable liquid containers for fuel are metal safety cans?

  • If hot work is occurring, have hot work permits been issued?

  • If hot work is occurring is fire watch established and protective methods in place?

Subpart G - Signs Signals and Barricades

  • Proper signage present as needed?

  • Are company vehicles protected from traffic exposures?

  • Are barricades and traffic control devices available and being used properly?

Subpart H - Material Handling and Storage

  • Are materials stacked and stored in an approved manner?

  • Are hooks with safety latches used when hoisting materials?

  • Are chokers, slings, chains, and other rigging equipment in good condition?

  • Rigging used properly?

  • Are areas below work locations properly secured for pedestrians and have alternate safe pathways been provided?

  • Materials dropped more than 20' have enclosed chute?

  • Employees are trying to minimize forces required for load handling? Using proper lifting techniques?

Subpart I - Tools - Hand & Power

  • Hand tools in good conition?(taped/cracked handles)

  • Power tools have required guards?

  • Power tool cords in good condition?

  • Tools used for intended purpose?

Subpart J - Welding and Cutting

  • Are cylinders, valves, couplings, regulators, hoses, and apparatus free of oily or greasy substances?

  • Are the cylinders properly labeled?

  • Are welding screens in use where employees may be exposed to harmful light radiation from arc welding? Have the regulators been removed and valve protection caps in place when not in use or in a stored condition?

  • Are the cylinders stored upright and properly stored?

  • Are cylinders transported in a manner to prevent them from tipping, falling, or rolling (approved cart or rack)?

  • Are welding leads protected, in good condition, and not lying in water?

  • Area free of flammable and combustibles within 35 feet

  • Fire protection equipment readily available?

  • Are bottles stored properly?

  • Proper set up of welding equipment?

  • Protection from arc flash provided?

  • Proper PPE for welder?

Subpart K - Electrical

  • GFCI or Assured Grounding Program in use?

  • Are electrical enclosures such as switches, receptacles, and junction boxes protected and provided with covers, doors, or guards? There are no exposed energized parts.

  • Extension cords in proper condition? (ground prong/electrical tape)

  • Extension cords protected from traffic?

  • Overhead lines on this site in proximity to work areas?

  • Lockout/Tagout methods correct to protect workers from energized circuits?

Subpart L - Scaffolds

  • Scaffolding inspected and all workers are aware of any hazards?

  • Scaffolding not over loaded?

  • Are guardrails and intermediate rails in place? Are floor boards secure?

  • Wheels on rolling scaffolding locked while in use?

  • Are all scaffold connections secure and built to specifications, scaffolding plum and square with cross-bracing?

  • Are workers protected from falling objects? Toe boards in place when >6ft?

  • Scaffolding fully decked?

  • Fall protection in use during assembly and dismantling?

  • Proper access provided?

  • Proper tie off inside aerial lifts?

  • Aerial lifts set up properly? Controls labeled and tested daily? Workers are properly trained?

  • Employees using Aerial Lifts properly, standing only on the floor of the basket?

  • Scissor lifts used properly, workers properly trained?

Subpar M - Fall Protection

  • Fall protection equipment appears to be in good condition?

  • Proper connection used for tie off?

  • Employees using equipment properly?

  • Hole and floor covers properly installed and protection for skylights in place?

  • Walking/Working surfaces have the strength and structural integrity to safely support workers?

  • Guardrails are installed correctly or Personal Fall Protection being worn to protect workers?

  • Is safety monitoring or traditional Fall Protection used for work outside warning line?

  • Is set back for warning line correct at 6' or 10' where mechanical equipment is perpendicularly traveling?

  • Flags are high visibility and spaced every 6'?

  • Warning line 34"-39" high and tied to stanchions?

  • Roof hatchway protected or closed?

Subpart O - Motor Vehicles and Mobile Equipment

  • Have operators been properly trained?

  • Employees wearing seat belts while operating?

  • Equipment is inspected daily and in proper operating condition?

  • Glass free from obstructions?(clean/cracks)

  • Back up alarms functional where view to rear is obstructed?

  • Modifications or attachments made to powered industrial trucks have approved design with re- rated plate as needed?

Subpart P - Excavations

  • Underground utilities located prior to any excavating operations?

  • Guardrails in place where walkways are 6' or more in depth along edges of the excavation?

  • Daily inspection by a competent person?

  • Proper access/egress for excavations 4' or greater and lateral travel no further than 25 feet?

  • Cave-in protection in place when excavation is 5' or more if not in stable rock?

Subpart Q - Concrete and Masonry Construction

  • Limited access zone established prior to the construction of a masonry wall?

  • Rebar caps in place or other barrier to protect from impalement hazards?

  • When cement, sand, and water mixture is being applied, head and face protection is being worn?

  • Masonry walls over 8' are adequately braced?

Subpart R - Steel Erection

  • Columns anchored by minimum of 4 anchor bolts?

  • Written notification that concrete has reached sufficient strength to support loads?

  • Hoisting cables are inspected daily and manufacturers tag is clearly marked?

  • Perimeter cables properly installed and clearly marked every 6' with high viz material?

  • No activities occurring below steel erection other than those involved in this activity?

  • Materials, equipment and tools not in use are secured?

  • Proper training provided by qualified person for all workers engaged in steel erection activities?

Subpart T - Demolition

  • During demolition, continued inspections by a competent person as work progresses?

  • Engineering survey in writing given to prepared by competent person?

Subpart X - Stairs and Ladders

  • Proper access provided for elevation change > 19"?

  • Stair systems have railing if 4 or more risers or > 30"?

  • Is the proper ladder for the job being used?

  • Is handrail properly installed?

  • Job built ladders constructed according to ANSI standards?

  • Are stairways free of obstructions?(including cords)

  • Is ladder at proper pitch? (4:1 ratio)

  • Ladder extends 36" above platform for access/egress?

  • Ladder secured from displacement?

  • Employees using 3 points of contact to climb?

Subpart Z - Toxic and Hazardous Substances

  • Are there activites which involve cutting, sawing, grinding, drilling, or crushing of concrete, brick & mortar, slate, granite, sandstone, etc.?

  • Workers are not exposed to asbestos, lead, silica or any other toxic or hazardous substances?

  • Do tools have a shroud and dust collection system?

  • Do tools have a water delivery system that supplies a continuous stream of spray of water at the point of impact?

  • Have work activities been coordinated to minimize silica exposure to all employees who are on-site?

Subpart CC- Cranes

  • Operator trained and licensed?

  • Crane inspections current?

  • Crane properly set up?

  • Swing radius barricaded?

  • Proper distance to energized power lines (>=20' unless using table A) maintained?
