Best Practices- Fatality and Serious Injury Prevention

  • The hopper on a Slag Plant shall never be entered when engulfment or entrapment conditions are present (i.e. built-up material)

  • Lockable gates shall be installed on all above ground feed hoppers to ensure positive lockout ability. Removable barricades shall be used on ground feed hoppers to prevent

  • All stored energy shall be controlled when cutting coils and any other objects.

  • A documented inspection process shall be accomplished prior to any cutting, burning, or welding.

  • The cab of pot carrier shall be aligned to face the direction of the hot slag during slagging off operations.

  • Always ensure that a Pot Carrier cannot physically place a tire over the slag pit wall when dumping a pot. All wheel stops shall contain guide flags to ensure proper tire alignment.

  • Adequate free-board of 12-18 inches shall be determined when hauling a slag pot. All pot carriers shall have a camera system to check free-board.

  • All Pot Carriers shall have a minimum of five lights in the approved locations to ensure 360° visibility at all times

  • A safety device shall be placed in front of the pit wall of all non-active pits demonstrating the proper pit to pour in.

  • Slag Pits shall be inspected for standing water before dumping a slag pot and the pit grade shall be as such to prevent standing water.

  • All Sites shall have and utilize a barrier that protects employees when adding air to tires, except when an outside company is used and contains the proper certifications (TIA).

  • All mobile equipment shall have a functional master lockout switch, except rental equipment.

  • Seat belts and safety glasses shall always be worn when operating mobile equipment.

  • All mobile equipment shall be turned off if the operator is more than 25 feet away or the operator will not have a direct line of vision of the mobile equipment.

  • Multiple warning signals shall be in place to alert equipment operators when the bed is in the up position.

  • All vehicles and mobile equipment shall honk their horn prior to any movement from a parked position.

  • All Levy-owned vehicles driving around the site shall have a functional strobe light system installed. If strobe lights are not installed, the vehicles must illuminate their hazard lights at all times.

  • Designated pedestrian walkways are required wherever there is potential for mobile equipment and pedestrian interface. Additionally, all pedestrian walkways below conveyors are required to be covered and labeled.

  • All hot loaders shall have pull-out hooks installed and all pot carriers shall have pull-out cables installed. The cable must be of adequate size and accessible.

  • All employees shall be trained annually on Dump Point and Stock Piling Safety.

  • A risk assessment (6-step or take-5) shall be accomplished for all non-standard tasks or any task that does not have a corresponding JBSA.

  • All on-site personnel are required to carry a functional 2-way radio or be escorted by someone with a functional 2-way radio.

  • Personal Safety Locks shall include the following: shrink wrapped photo-ID’s, the employee name, company name, and a phone number to contact the employee.

  • All personnel are required to wear reflective tape on hard hats and a reflective shirt/vest/jacket (outer most layer reflective).

  • A remote jump start system shall be implemented for all 12 and 24 Volt batteries.

  • All Slag Plant Screens shall have approved tie-off points and adequate access to those tie-off points allowing 100% tie-off capabilities

  • All fall protection equipment used at the plant shall include double lanyards. All lanyards must be self retractable.

  • All hot loaders shall have retrieval cables installed on the back of the machine. The cable must be of adequate size and accessible.

  • All loaders shall have a blue approach light mounted on the rear.

  • All picking stations shall have a tethered e-stop when an employee is working alone on the platform.

  • All pot carriers, drop ball cranes, and hot loaders shall have ballistic glass in the approved locations.

  • All vendors/contractors shall sign in at the site's office and sign off on site safety rules prior to proceeding to any other location on site. Vendors/contractors shall also sign out prior to leaving the site

  • All haul trucks not protected with hand rails shall be equipped with an additional fall prevention method/device

  • All at-risk overhead power lines on-site shall have a visual indicator on or near the lines.

  • All hot pits shall have an engineered approved and documented watering practice.
