General Safety

  • Was there a filled and signed JHA addressing all hazards observed onsite?

  • Does the foreman have a record of JHA's logged?

  • Are there inconsistencies in the JHA's such as lack of task detail, or similar?

  • Did you address the JHA inconsistencies with the supervisor?

  • Is all required PPE being used?

Fall Protection

  • Are all workers involved with at- heights risks using a fall protection system?

  • Are the fall protection systems used adequate for the task? (Ex. Fall clearances, proper tie off locations, elimination of swing falls, etc.)

  • Are tags and other markings legible and in place?

  • Was the equipment tagged as unusable and removed from service?

  • Are workers performing visual inspections prior to each use of the fall protection systems? (Ask workers involved with such tasks.)

  • Have workers been trained in fall protection use and inspection?

  • Has training been scheduled with the appropriate parties?

  • Is the Foreman trained to recognize fall related hazards and unsafe conditions?

Steel Erection

  • Is there a proper method of communication between the crew members? (Ex. Radio communications, hand signals, etc.)

  • Are all workers receiving panels using a portable ladder to access the truck beds?

  • Are riggers communicating with the crane operator and nearby workers to ensure all unneccessary parties are out of the fall zone?

  • Are whistles, taglines and the crane's horn being used to signal when hoisted loads are in transport?

  • Are erectors receiving the loads via tagline? (Note: Coach against hand- handling of loads when unnecessary.)

  • Are erectors over- reaching for the loads as they receive them?

  • Are panels being secured into place before the crane is disconnecting from the panel?

  • Does the lead erector/ foreman have clear communication with the crane and crew once the panel has been cleared for disconnection?

  • Are workers using ladders or MEWP's to safely access their elevated work position?

Crane

  • Does the crane operator have the appropriate inspection forms readily available and filled?

  • Does the crane have a properly set swing radius barricade?

  • Are there any hazardous obstructions or unnecessary workers within the crane's swing radius?

  • Does the crane operator have clear communication with the crew?

  • Are there any visual deficiencies with the crane? (Ex. Leaking oils, excessive smoke, lack of markings on the cranes body?)

  • Is the crane operator current with their NCCCO certifications?

Rigging

  • Are all riggers trained to perform their duties?

  • Has supervisions been notified and training been scheduled?

  • Does all rigging equipment have affixed and clearly legible identification tags?

  • Has the rigging been tagged as unserviceable and removed from service?

  • Does any rigging equipment show signs of overuse or deficiency? (Ex. Abrasion, kinking, knotting, missing tags, etc.)

  • Has the rigging been tagged as unserviceable and removed from service?

  • Are riggers using the whistles or audible communication tools to ensure the site recognizes when a love load is in transport?

  • Are riggers visually inspecting equipment prior to each use?

  • Is all rigging being properly stored?

Ladders

  • Are all ladders being used of the appropriate type? (Ex. A- frame ladders cannot be used in place of extension ladders.)

  • Do ladders being used show signs of excessive wear, rust, or damage?

  • Are ladders being used outside of the marked capacity ratings?

  • If an extension ladder is being used to access an upper landing, is it 3" past the surface?

  • If an extension ladder is being used, is it staged at the appropriate 4':1' safety ratio?

  • Are all ladders equipped with slip resistant feet?

  • Is work being performed atop the ladder's top step, or beyond the manufacturer recommended height?

Welding

  • Are all welding leads free from damage within 10' of the electrode holder?

  • Are fire extinguishers posted where each welding operation is taking place?

  • Have accommodations been made to obtain the needed fire extinguishers?

  • Is the fire extinguisher properly inspected with an appropriate tag?

  • Are welders using the required PPE when preforming hot work?

  • Are all gas cans used onsite of the appropriate type? (Ex. Safety cans.)

  • Do all handheld grinders have their guards in place?

  • Are all grinders operated with an integrated deadman switch?

  • Are workers wearing adequate face protection when grinding?
