EVALUATION

General Site Access Conditions - Inductions and access to site

  • The Scope of Work for the Worksite has been confirmed by the PIC

  • The TPO has obtained the Track Possession Authority

  • The TPO has issued the PIC with a Permit to Occupy Authority Limits

  • The PIC completes a Worksite Briefing/Protection Plan Form for their Worksite

  • The PIC briefs all their personnel at their Worksite on the Worksite Briefing/Protection Plan. GRA Pre-start meeting is completed and all workers signed on.

  • Lock system operational and maintained

  • Once the PIC has Locked On to the Lock Station, all personnel place their Personal Isolation Lock on the Lock Station and enter their FULL name in BLOCK letters, Personal Isolation Lock identifier and initials on the Worksite Lock On/Off Form

  • The PIC shall ensure that red flags are appropriately secured to the track at the limits of their Permit to Occupy or closer to the physical Works (where applicable) and ensure that all red flags that are placed on track are accounted for on the Worksite Lock On/Off Form

  • Barricades/cones/tape/temp. fences in place to prevent public access

  • Adequate lighting/ventilation

  • Safety and warning signs in place (e.g. Hot Works)

  • PPE signs in place

  • Site clean

  • Access ways clear of debris and tools?

  • Communication means established i.e. UHF radio and mobile phone

  • Site visitors inducted and signed on to Pre-start and/or Visitor Logbook

  • Personnel tickets available on site for the task being undertaken

HSE Management System

  • Emergency/Rescue procedure in place

  • SWMS adequate, reviewed and implemented on site by workers. Subcontractor SWMS to be verified against the GRA SWMS Checklist (GRASFM24)

  • Pre-Start meeting and Toolbox Talk records available

  • Permits/Approvals to work obtained (e.g Hot Works, Permit to Occupy)

  • GRA Project Safety & Environmental Management Plan available for consultation

  • People verified competent to perform tasks

  • Adequate Supervision for all the tasks performed

Mobile Plant

  • The Operator of any Equipment/Vehicles shall place an Isolation Lock on the appropriate Worksite Lock Station for the Equipment/Vehicle and enter the identifier for the Equipment/Vehicle, Isolation Lock Identifier and initials on the Worksite Lock On/Off Form

  • Each PIC shall confirm all Personnel/Equipment/Vehicles have locked on (verified by the number of padlocks matching the number of entries on the Worksite Lock On/Off Form) before authorizing commencement of Work

  • SWMS acceptable for work being carried out

  • Plant condition suitable for use

  • Daily pre-start checklist completed for mobile plant

  • Maintenance record of vehicle valid

  • Drivers duty record within limits

  • Barricades/cones/tape/temp. fences and signage in place

  • Noise and Exhaust controlled

  • Seat belts fitted, as required

  • Safe working loads indicated

  • Prestart checklist completed for Mobile Plant
