Is form work straight and true - properly braced?

Is panel square? -Maximum 1/8” in 10’ (non-cumulative) per SPEC

Are punched openings square and in correct location?

Is reinforcement steel correct size and spacing?

Is reinforcement steel properly chaired?

Are reinforcement steel lap coverages sufficient?

Are slab dowels, stirrups, hairpins etc installed per reinforcement drawings and properly tied?

Are embeds correct size and in correct location per panel embed drawings and properly secured/squared and leveled?

Is form liner placed where required and in good condition?

Are reveal and chamfer strips properly placed, straight and secured?

Are door frames, where required, squared and straight per construction documents?

Are electrical pathways installed and protected where required?

Are required blockouts installed properly?

Is Horizontal/Vertical sleeving installed where required and protected?

Is formed area clean and free of debris?

Is panel safe to pour?
