TLW Scaffold Checklist

Are scaffold components and planks in safe working order?

Have competent persons been in charge of erection?

Is scaffold tower plum?

Is the tower height less than 4x the minimum base width?

Are sills in place and proper size?

On mobile scaffolds, are the wheels locked while in use?

Has proper access (ladder) been provided?

Is the tower fully braced on both sides?

Has the scaffold system been tied or guyed?

Have all work platform areas been fully guard railed?

Is the platform fully decked?

PFSA and guardrails on two-point suspension scaffold?

Are scaffold base plates on solid ground?

Has fall protection been provided for all fall exposure 10' or more?

Are platforms and walkways at least 18" wide?

Do platforms 10' or less extend over their support 6-12"?

Do platforms greater than 10' extend over their support 6-18"?

Is there proper clearance between scaffolding and powerlines?

Is scaffold platform clear of excess debris?

Have all precautions been taken to prevent falling objects?

Is a competent person inspecting the scaffold before use?

Have all employees using scaffolding been properly trained in hazards associated with that scaffolding?
