Trip Performance Standards

  • Was the entire crew ready to go by 30 minutes prior to the scheduled takeoff time of each leg? <br>If "NO", then briefly describe the situation and what caused the crew to be late in the notes.

  • Was the cabin safety check completed and Flight Attendant seated by 10,000 ft on all legs?<br>If "No", please explain in the notes why the check was not completed or Flight attendant not seated by 10,000 ft.

  • Were all approaches stabilized on every leg of the trip?<br>Stabilized Approach Criteria @ 1000 FT AGL IMC or 500 FT AGL VMC:<br>ON SPEED, GEAR DOWN, FLAPS LANDING SET, POWER STABILIZED, SPEED BRAKES RETRACTED, 3 DEGREE GLIDE PATH<br>If "NO", then briefly describe the situation and what caused the approach to be unstable.

  • Was the trip flown without any errors caused by communication breakdowns or misunderstandings (FOS, Briefings, CRM issues)? <br>If "NO", then briefly describe the situation and what errors occurred.

  • Was there a comprehensive Pre-trip briefing and Post-trip debriefing conducted that included all team members concerning the entire trip. If "NO", then briefly describe in the notes the situation and why the trip was not briefed to standards. Also, include which team members were not properly briefed.

  • Did the crew hold each other accountable for all errors made on the trip? If not, then please explain in the notes the circumstances in that members were made or not made accountable.

  • Are the any other crew performance issues (Good or Bad) you would like to highlight?
