  • Confirm the Contractor/s have been signed in at the Venue on the site access register and given a localised induction/instruction in the venue.

  • Does the contractor have current SWMS documentation on hand with them onsite for the task being completed? (attach photo/evidence)

  • If the contractor is completing work near overhead or underground powerlines have the appropriate pre start documentation been supplied to SCI & the venue/event site for verification

Venue/Event Site Management Section

  • Is there a WHS Management Plan onsite

  • Have all subcontractors been inducted (evidence required) attached photo of signing into site register which acknowledges induction

  • Can contractors present relevant licences and permits <br>(crane, dogging, EWP, DBYD etc)

  • Are barricades erected around work area?

  • Is the venue/event site clean and tidy and free of trip hazards?

Traffic Management

  • Are traffic management plans available and approved?

  • Is traffic management in place as per the traffic <br>management plan (signage, bollards etc)?

  • Has pedestrian and cyclist traffic been identified and <br>managed?<br>

  • Can traffic controllers present their licences?

Hazard Identification and Risk Management

  • Is there evidence that hazards are identified, <br>assessed, controlled and documented (confined <br>spaces, excavations, fall from height etc)<br>

  • Are SWMS, JSEAs, operational guidelines and/or event/venue/client risk assessments available, <br>site specific and relevant?

Work at Height

  • Is any work conducted at height (≥ 2m)?

  • Is the risk of falls controlled appropriately (use of platform ladder<br>scaffold, EWP, travel restraining / fall-arrest etc)?

  • Are relevant SWMS/JSEA in place?

Noise

  • Does the venue or event have a noise management plan in place or are they limited by DB levels aligned with Liquor license

  • Is noise protection required and if so, is it available and <br>being used correctly? ie ear protection or keeping doors to spaces closed to control sound bleed

Emergency Preparedness and Response

  • Have emergency situations on <br>site been identified and have evacuation response plan?<br>

  • Is emergency equipment (fire extinguisher, first aid kit, <br>spill kit) available and in date?<br><br>

  • Is there a dedicated first aider onsite (provide name of individual)

  • Is the Frist aid kit suitable for the site

Asbestos

  • Has Asbestos been identified on site?

  • Are controls in place to control the Asbestos? e.g. <br>signage and appropriate PPE

Confined Spaces

  • Are confined space activities being performed?

  • Are relevant Safe Work Method Statements (SWMS) <br>and permits in place?

Excavation

  • Are excavation activities being performed?

  • Are relevant SWMS/JSEA and permits in place?

Electrical

  • Is electrical equipment tested and tagged?

  • Are Residual Current Devices (RCDs) in use?

  • Are relevant SWMS/JSEA in place if electrical contractor is performing work at venue?

Mobile Plant and Equipment

  • Is mobile plant being used on site?

  • Is there evidence of pre-start checks?

  • Is there evidence of Verification of Competency <br>(VOC)?

  • Are relevant SWMS/JSEA or operational plant guideline in place?

Fatigue/Load Restraint/Mass Dimensions

  • Have rosters considered LCC fatigue management directive

  • Have the staff onsite taken the required breaks to ensure fatigue compliance

  • If transporting equipment or deliveries coming to venue/event site are the loads properly secured or contained, with the <br>restraint arrangement providing enough clamping force to prevent movement during transit<br>

  • Are the restraints in good condition (e.g., ratchets and <br>straps undamaged with no signs of unacceptable <br>wear)?

  • Is the load weight and dimensions within the specified vehicle load mass limit.

Personal Protective Equipment

  • Is appropriate personal protective equipment (PPE)<br>available in line with contract conditions and being <br>used and maintained correctly?

Lifting Gear

  • Is all lifting gear (chains and slings) tagged, current <br>and in good condition?

Hazardous Substances

  • If hazardous substances are in use at the venue or event site is a <br>Safety Data Sheet (SDS) available?

Other Hazards

  • Are ladders in good condition and fit for purpose (no <br>domestic ladders on site)?
