  • 1. The worker’s employer’s name (ie Contractor, sub-contractor’s company name).

  • 2. A photo of the worker’s employer class B asbestos removal license clearly displaying company name, license number and expiry date.

  • 3. Name, number and location of the available supervisor during the time of asbestos removal.

  • 4. A photo of supervisor’s asbestos removal certificate.

  • 5. Proof of the workers identity (i.e. driver’s license) cleary displaying the name and the photo of the person performing the asbestos removal.

  • 6. A photo of worker’s Class B license to remove non-friable asbestos via an accredited training course (or state/territory regulator approved equivalent) as a minimum.

  • 7. A photo of worker’s white card.

  • 8. A photo of the community notifications if applicable, resident notifications (can be letter attached with the address on the letter).

  • 9. A photo of the Asbestos work area, exclusion Zone and Decontamination Zone

  • 10. A photo of the Asbestos removal area (Guards/Physical barrier around the excavation)

  • 11. A photo of the Signage used during the works (Danger Asbestos Removal).

  • 12. A photo of pedestrian traffic management used during the works. Signs, barriers & hats

  • 13. A photo of the worker wearing correct PPE to show it has being used and worn correctly i.e. taped at wrist and ankle, hood on.

  • 14. A photo of all waste showing it is being double bagged or plastic wrapped and sealed in 200micron plastic. Bags goose necked and sealed with duct tape.

  • 15. A photo of bagged asbestos in a certified vehicle for carriage (Photo/evidence, the bagged asbestos is being transported safely to the licensed waste facility).

  • 16. Photos of equipment being used for the ACM work, and where that equipment is normally stored?

  • 17. Scanned Dockets indicating the date and location of the authorised licensed waste facility or holding area where the asbestos was disposed of. (Telstra requires contractors removing ACM to dispose of the waste in accordance with relevant State / Territory legislative requirements that apply to the location where the waste was generated.)

  • 18. A photo of the Site clean-up & reinstatement showing the work area and exclusion zone.

  • 19. A copy of the Site specific SHEWMS for the ACM activity.

  • 20. Preliminary soil screening assessment or certified result. This will determine how the soil is managed.
