Senior Management Observations

Instructions

  • Note: The content of this Safety Walk is based on WML critical risks which are: Confined Space, Energy Isolation, Lifting/Suspended loads, Working at Height, Traffic Management, Hazardous Substances, Mobile Plant , Maintenance & Inspection.

  • Progress through the applicable sections, answering each question. When an item is non-compliant, be sure to add notes and photos as evidence.

Maintenance and Inspection

  • All maintenance activities including working at height, confined space, electrical isolation etc. are permitted, identified and controlled.

  • Work and storage areas are clean and tidy?

  • Equipment and tools (chains, gantries, cranes and other lifting gear) are appropriately stored?

  • Equipments (lighting fixtures, machine guarding, barriers, etc.) are maintained in good working condition and in place.

  • Signs indicating required Personal Protective Equipments are in place.

  • Signs indicating eyewash/safety shower, emergency exits, fire extinguishers, etc. are in place?

Emergency Response

  • Are those onsite aware of what to do in the event of an emergency?

  • Emergency equipment is readily accessible - easily found, not obstructed in anyway (e.g., eyewash/safety shower, first aid kits, fire extinguishers).

  • Emergency routes and emergency doors are kept clear of any item and fully accessible. Emergency doors are not locked.

Personal Protective Equipment (PPE)

  • Is PPE used appropriately throughout the site? (look for; safety glasses, seatbelts/lap belts on buggies/mobile plant, hearing protection etc.)

Hazardous substances

  • Are Safety Data Sheets (SDS) current (issue date within 5yrs) and readily available at key locations throughout the site (red boxes)?

  • Hazardous substances are stored appropriately (secure, signage erected, secondary containment, incompatible substances separated, PPE available)?

  • Can spills be managed effectively (are spill-kits available and in good condition)?

Traffic Management

  • Is traffic management being managed effectively is there anything we can do better (e.g., internal building, lighting within shared thoroughfares)?

  • Are roads/pedestrian walkways and shared thoroughfares kept clear of obstructions?

Mobile Plant

  • Is mobile plant being operated safely - (safe procedures followed, e.g., rules of the road, maintaining separation between mobile plant (e.g. forklift), truck drivers and foot traffic etc.).

Working at height

  • Is there adequate fall protection controls in place (barriers, scaffold, cordoned off work area, fall restraint equipment, harnesses, lanyards, certified anchor points and safety netting etc.?

  • Has a permit to work been issued and does it cover off foreseeable hazards and controls adequate to minimize associated risk.

  • Are those using fall restraint equipment, height safety trained and familiar with rescue procedures.

  • Are mobile elevated work platforms (MEWP) being used is the operator trained and competent?

Lifting/suspended loads

  • Chains, strops, gantry cranes, cranes and other lifting gear is fit for purpose (tagged and tested)?

  • Is there an approved lift plan

  • Is the lift area cordoned off to prevent entry into fall zone.

Energy Isolation

  • Is there lock out tag out procedure in place?

Confined Space

  • Has a confined space permit to work been completed and are hazard controls adequate?

  • Has a confined space emergency response plan been completed and are those involved familiar with the procedure.

  • Are workers trained in confined space and signed off as competent.

  • Is there an air monitoring process in place?

  • Is the task adequately resourced with stand-by workers and safety equipment?

Overall Notes
