1. Have alternative ways of carrying out the work been considered?

2. Has it been assessed whether a fall will be arrested before hitting the ground/other structure? Right lanyards in use? Static line deflection taken into account?

3. Does Hazard ID cover work at height, permit and control measures?

4. Permit to work at height (or mancage permit) complete and signed off?

5. Rescue plan complete, signed off and adequate?

6. Emergency numbers available and clearly displayed?

7. Have anchorage points been rated, appropriate for the load and inspected?

1. Are we using tool lanyards? Is there a risk of falling objects striking people below? If yes, is the work area cordoned off, signage in place to prevent unauthorised entry? Are all exclusion zones suitable? Have all drop zones been identified?

1. All equipment (JLG, MEWP, rescue kit etc.) needed for rescue available? Is a trained person available to carry out the rescue?

2. Is the rescue kit available and stored correctly?

3. Is all WaH gear (harnesses, lanyards, static lines, mancages etc.) stored properly? No equipment exposed to conditions causing deterioration?

4. Is WaH gear in good condition and not damaged? Are karabiners and energy absorbers functional and undamaged?

5. Is all WaH gear tagged and tested?

6. WaH equipment (harnesses and lanyards) is designated as authorised use only?

1. Has the scaffold been tagged and inspected?

2. Is scaffold and EWPs adequate and erected correctly? Toeboards, guardrails, midrails and netting in place? Is the access clear?

3. Are surfaces/ground stabile for safe support of scaffolding or working platforms?

4. Is the work area (platforms, walkways etc.) clean and free from obstructions?

5. Are ladders in good condition, not bent or damaged and set up correctly?

6. Have daily pre-start checks (pink slips) been done prior to commencing work?
